Warning! Change your name in the Diploma and ECFMG

If you can't change the name in your passport then you have to stick to that name for the rest of your career life in USA.

In that case you have to take the hassle of contacting your med school and ask them to change the name in your medical school and Diploma and if they refuse at least ask your dean to write a letter that the person holding this Diploma is the same person named in this passport and then contact ECFMG and change your name with them by using Form 182 which is available here http://www.ecfmg.org/forms/form182.pdf

You might think that this is a long process and will slow your registration process, but this is the only way to tackle your problem in order to avoid devastating consequences later on in USA.

Even if the Prometric officials allowed you to take the exam by accepting a document other than a passport, you won't get away later on when you apply to residency programs and getting your social security number and carrying on with you life in USA.
