Form Resending Form 186

I have got the following message from ESFMG:

The Educational Commission for Foreign Medical Graduates (ECFMG) is unable to process the Certification of Identification Form (Form 186) for your on-line application submitted December 3, 2010 for the reason described below. ECFMG cannot determine that your Certification of Identification Form (Form 186) was sent to ECFMG by the medical school official who signed the Certification of Identification...
I called them saying that Form 186 was send actually by the medical school. They said that there was no return address on the envelope.

In this letter they ask me to sign the form in the presence of an authorized official of my medical school.:

Therefore, to be registered for the USMLE Step you requested, you must reapply by submitting a new Certification of Identification Form (Form 186) completed in its entirety.
Sign and date the Certification of Identification Form (Form 186) in the presence of an authorized official of your medical school. The medical school official must sign and date the Certification of Identification Form (Form 186) and provide his/her name, official title, and the institution name. The signature of the medical school official must match an authorized signature that is on record at ECFMG. The official must place the medical school seal so that it covers a portion of your photograph and the Certification of Identification Form (Form 186). The signature of the medical school official must be current; the official must have signed the Certification of Identification Form (Form 186) within four months of the date that it is received at ECFMG.
Unfortunately I can not sign the form in the presence of an authorized official of my medical school. I live and work in Canada, the school is in Romania (on the other side of the planet Earth.
There is no word in the letter saying that I could sign the form in the presence of a consul, notary public, commissioner of oath...

This is how I did with my first Form 186: I signed FORM 186 - CERTIFICATION OF IDENTIFICATION FORM - NOTARY PUBLIC AND MEDICAL SCHOOL OFFICIAL in front of a notary public, I sent it to my school, then school sent it to ESFMG.

What to do now? May I sign FORM 186 - CERTIFICATION OF IDENTIFICATION FORM - NOTARY PUBLIC AND MEDICAL SCHOOL OFFICIAL and send it to my medical school again (with the note that the return address should be included on the envelop), or should I fly to Romania to sign FORM 186 - CERTIFICATION OF IDENTIFICATION FORM - MEDICAL SCHOOL OFFICIAL in the presence of a medical school official?

Default hello

The best way is to sign the 186 )from U.S Embassy and then send the papers to your friends in Romania
Till him to do all the instructions so they will do it step by step
you should print the address of ECFMG on the envelop and send it by your medical school(put the adress of your medical school as a sender)

Ask ur med school to put office stamp outside the envelope...this is what I did from Russia.

Thank you both guys. I sent the letter with FORM 186 - CERTIFICATION OF IDENTIFICATION FORM - NOTARY PUBLIC AND MEDICAL SCHOOL OFFICIAL to my school. I did that before moath replayed to my question. Fortunately I have spoken to my friend and he is going to go to the school to check up for the letter to have all addresses an all stamps on it as kdm0101 advised me.

@ gigea: I wanted to know. when did you get that mail, within 2 weeks of sending your document or after a month or so?
