Warning! Form 182 and changeing date of birth

Hello everyone

While trying to submit an application for ECFMG certification I discovered that I've entered a wrong date of birth.

I filled the 182 from with my ECFMG ID#, name, current date of birth, the corrected date of birth and my signature. I attached a photocopy of my passport with the form (pages 3 and 4 only) and then faxed them.

Yesterday I received an email informing me that they have received my request to ((change my name)) and that my faxed document was not legible.

So what did I do wrong:
1. Should I fax the whole 4 pages of the form?
2. Is my passport invalid?
3. Did I miss anything while filling the form? they said that I requested to ((change my name)) while I was trying to change the date of birth.

Any help is really appreciated. I don't want to resend it and wait just to find out that I missed something again. It took them 7 days to respond and that's already too long.
