Arrow From Registration to Scheduling Permit; Step by Step Process

Hi,

This forum helped me big time understanding the process. So just thought of sharing my experience, that may help others in same boat

I am in US right but I am an IMG from India.

I created my ECFMG ID back in September and completed USMLE application in December.

I used 186 (Notarized one, as I am already in US).
Sent 186, 187, 344 and 345 back home.

My family back home managed to get necessary sigsn from my med school and they sent back all of these forms (make sure 2 copies of 345) to ECFMG.

ECFMG received these on Jan 23 but they did not update the system until Jan 30. Though I got mail on 30th, online system showed receipt date 23 only.
That day I got mail confirming receipt -

We are pleased to confirm that your Certification of Identification Form (Form 186) has been received by ECFMG. Your USMLE/ECFMG Identification Number is listed in the Subject line of this message. You must use this number in all communications with ECFMG.

ECFMG typically processes on-line USMLE applications within 3 weeks of receipt of both the on-line part and the Certification of Identification Form (Form 186). You may track the status of your application by using ECFMG's On-line Applicant Status and Information System (OASIS). Access OASIS on the ECFMG website at oasis.ecfmg.org.

Next on Feb 7 I got mail confirming Acceptance of form -

--------------------------
Your Certification of Identification Form submitted 12/**/2011 has been accepted. This form will remain on file and be valid for a period of five years. The validity dates are as follows: 02/07/2012 through 02/07/2017.

The Certification of Identification Form is processed independently of the on-line application. Acceptance of the Certification of Identification Form does not automatically guarantee acceptance of the application. Once your application has been reviewed, ECFMG will notify you of the outcome of your application.

Very same day I got another mail -

The processing of your USMLE Step 1 application is complete. You have been registered for the February 1, 2012 - April 30, 2012 eligibility period in the United States and Canada testing region. ** Please see BELOW for details on the option to submit a request to have the exam results for this Step 1 withheld from your medical school**.

The eligibility period assigned to you for this exam may be different from the one you selected on your application for one of the following reasons:

- If you selected an eligibility period that has already begun and your application was not processed in time to assign the eligibility period you selected, you have been assigned the next eligibility period.

- USMLE policy states that you may not take the same Step more than four times in any 12-month period. If you have already taken this exam and the eligibility period you selected on your application does not comply with these requirements, you have been assigned to the first eligibility period that does meet these requirements.

The National Board of Medical Examiners(r) (NBME(r)) will issue your permit, which contains information on contacting Prometric to schedule a testing appointment. (If your assigned eligibility period begins more than six months from now, your permit will be issued approximately six months before the beginning of your assigned eligibility period.) Only electronic permits will be generated for Step 1/Step 2 CK; applicants will access their electronic permits using ECFMG's Interactive Web Application (IWA). You will not receive a paper permit by postal mail. In about one week you will receive another email from ECFMG to notify you that your permit is available. This message will include instructions for accessing the electronic permit using IWA. Although your permit will be issued by NBME, you should contact ECFMG by email at info@ecfmg.org or by phone or fax if you have questions or concerns about this document. You must print a copy of the electronic permit and bring it to the test center on your exam date. Your permit is a very important document, and you should be careful not to lose it. You cannot schedule your testing appointment or take the exam without it. If you lose your permit, you may print another one by using IWA. If you cannot take a Step during your assigned three-month eligibility period, you may request an extension. There is a fee for this service. You may request an extension on-line using ECFMG's Interactive Web Application (IWA) or by submitting to ECFMG a completed Request to Extend a USMLE Step 1 / Step 2 CK Eligibility Period (Form 176). For information, access IWA or Form 176 on the ECFMG website.

and then finally yesterday I received my scheduling permit -

Your STEP 1 SCHEDULING PERMIT IS NOW AVAILABLE on the ECFMG Interactive Web Applications (IWA) website.

YOU MUST PRINT YOUR SCHEDULING PERMIT AND BRING IT WITH YOU TO THE TEST CENTER ON YOUR SCHEDULED DATE. You will not be able to take the test if you do not bring your Scheduling Permit to the test center. Note: You will NOT receive an orange Scheduling Permit for this exam in the mail.

To access your Scheduling Permit, go to IWA at https://iwa2.ecfmg.org, login, and select "Print/Reprint your USMLE Step 1 Scheduling Permit." Your permit will open in a new browser window. Use your browser's "Print" function to print the permit.

Bottom line you just need to follow the process and ECFMG does its job very efficiently.

From what I understand now they will send my Degree certificate with Form 345 directly to medical school back home. (I should receive another mail at that point) and then school needs to submit transcripts directly.

In the meantime I am good to give Step1 and Step2 CK.

Default confused

hey there ,
I am an undergrad and I have been getting pretty much confused with the forms and numbers and all ...I got 183 and 344 would i get what they give me to print and fill or is it that i would go on scavenging for other forms somewhere on the site that i don't know about

Form 345 is for graduates only. Since you are still a medical student you won't get form 345.
So, yes, you are only required to do Form 183 and and Form 344.

Warning! question

thank u bharadu for sharing
I have a question ... I sent my papers back home... and I'm afraid it will take more time than I expected ..
my question is if I chose an eligibility period and my papers weren't done by that time... do they change the eligibility period automatically to the next one ??? sorry I didn't understand this point !!

yes they change the eligibility period automatically. In fact I had chosen Jan to March .. but since my papers were not on file by Jan 24 they bumped me to Feb-April..

If you want peace of mind you can call ECFMG as well. I did call them on Jan 25 and thats what they told me

oh thank U .. It's just today I knew it may be a delay ... but I was reading now and discovered that 186 form must have been signed within 45 days other wise it may be rejected !!!! the problem is mine has been signed in September in the US ... and just yesterday in my medical school ! I know I have to call. but any idea about that ?!!!!!

Unfortunately never heard about 45 day regulation .. your best bet is to call ECFMG and confirm ..

No u cannot .. There is a form 186 should be signed by u here in presence of a notary public.. And the same form should be signed there by ur medical school and then sent back to ecfmg .. Try to find a friend there who u can send him the papers and the money the school needs to do the shipping .. That what I did .. And make sure to send them ur diploma or a certified copy .. U need to have somebody there to compete these prossess !

Thank you for clarifying some confusing stuff. But I still have a few questions.
I'm still a student so apparently I wont have to get the form 345 right?? So do I have to get forms 183 or 186 signed? I didn't see any of those forms in the USMLE website. Where do I find them? Are they available only after you choose a test date?
One more thing, how long does all this form generating and form signing stuff take? How many months in advance do we have to apply for all this stuff before our chosen exam date?
Sorry if these questions seem silly

its not 45 days ,it should be signed within 4 months.

i'm an IMG too, however, my school doesnt participate in EMSWP and i'm not in the US yet,which form of the 186 forms shall i choose:

1- 186 form which caklled the form for all medical students.(option number one in ECFMG website).

2- 186 form,which is signed by both notary and med officials.(option number2)

3 is excluded as it's made for colleges which participate in EMSWP.

So I have to sign the 186 before sending it back home and notarize it here? I was thinking about just sending the pdf's and letting my family print them and take them to my school to make it faster.

That won't work .. 186 has your photograph also it will have notary seal so PDF would not work .. You have to send original .. Global express takes just 3-4 days from USA to anywhere cc so not sure how much time you save by sending PDF .. Good luck

"Because your medical school participates in EMSWP, you are able to complete either a "Certification of Identification Form (Form 186) - Certification by Medical School Official" or a "Certification of Identification Form (Form 186) - Certification by Notary Public.""

So I went with:

If you complete a "Certification of Identification Form (Form 186) - Certification by Notary Public":

Sign and date the Certification of Identification Form (Form 186) in the presence of a Consular Official, First Class Magistrate, Notary Public, or Commissioner of Oaths. The official must sign and date the Certification of Identification Form (Form 186) and provide his/her title. The official must affix the official seal in the designated section of the Certification of Identification Form (Form 186). The signature of the official must be current; the official must have signed the Certification of Identification Form (Form 186) within four months of the date that it is received at ECFMG.

The following must be sent to ECFMG:

Original, signed Certification of Identification Form (Form 186).

Two photocopies of medical diploma (if not previously submitted) and official English translation of medical diploma (if diploma is not in English); one current, original, full-face, passport-sized, color photograph (this photograph is in addition to the photograph that applicants must send with the Certification of Identification Form); the ECFMG Medical Education Credentials Submission Form (Form 344); and two copies of theMedical School Release Request (Form 345).

If applicable, two copies of an official transcript issued by any school or institution from which you transferred credits to the medical school that awarded your medical degree (if not previously submitted); official English translation of transcript (if transcript is not in English); and the ECFMG Medical Education Credentials Submission Form (Form 344). If you are also submitting copies of your medical diploma, you only need to submit one Form 344. See information on transfer credits and medical education credentials in the applicable edition of the ECFMG Information Booklet.

Any additional documents that are required to complete your application.

A completed IWA Document Submission Form (Form 187) if, in addition to your Certification of Identification Form (Form 186), there are any additional documents required to complete your application.


So apparently, I can do everything from here (US). I am just waiting for the translation of my medical diploma to arrive.

Hi guys im a IMG from india. I have completed my course 1 wk back. im planning to write step1 by august. Itll take 2 mths for me to get my medical diploma. Actually now im really confused bcse now im a graduate but i dont have a medical degree. Can anyone pls guide me wat forms to send.
