Default Stepwise guide for ECMFG certification

This post is based on the older post from this forum by Saibo regarding the steps for ECFMG certification. There are a few differences with the updates to the certification process given the time. Hopefully it will be helpful for people who are starting the process.

1. USMLE/EFCMG ID - First step is to obtain a eight digit ID number via ECFMG website. Its pretty straightforward and takes about 7-10 business days for ECFMG to send you the ID via email. All future communications to ECFMG (including the photocopies of documents like degree, passport, etc) should have your ID number written.

2. Application for ECFMG certification - once you have the ID/password, you can log in to IWA and complete the application for ECFMG certification. You have to fill up personal/medical school information and pay a fee of 65$.

3. Step application - You can now apply via IWA for step 1, 2CK or 2CS. Esentially you begin the exam application online, fill up details including your medical school enrollement, internship and graduation, clinical clerkships, choose your desired eligibility period and pay the exam fee. Depending on whether you are a student or graduate, whether or not your medical school participates ECFMG web portal for status verification you will be issued either a form 183 or form 186 (students from school not participating in online status verification get form 183 while graduates, students from schools in online status verification get form 186). You have to get the form 183 or form 186 signed by the authorized ECFMG representative from your school.

4. Send the documents to ECFMG - If you get a form 183 or if you are a student with form 186, you will only have to send the form. If you are a graduate, you will be usually sending form 186, 2 copies of form 345 (medical school release request), photocopy of your degree. (You may have to send a letter from your medical school signed by the authorized official verifying your name if your name in your passport/ECFMG record does not match that on the degree/diploma. Additionally, if you have graduated but have not yet recieved your degree and wish to apply for the exam, you will have to send a letter from your school stating that you have completed all the requirements for graduation including the date of graduation and when the degree is expected to be issued. Once you get the degree, you need it to send it to ECFMG). Note that you have to send a form 183 each time you apply for an exam if you are a student while form 186 has to be sent only once and has a 5 year validity (for a subsequent step exam you have to just complete the online application if you have a valid form 186 on file)

5. Scheduling permit - Once ECFMG processes the form 183 or form 186 which takes about 3-4 weeks, your scheduling permit will be issued. If you send form 186, you need to careful with the name issue described above as ECFMG will accept form 186 but reject the exam application if your name on degree and ECFMG record do not match and you have not sent a letter verifying the name. Another thing people usually get confused is between exam application and credentials verification. When you send the degree to ECFMG, it starts the process of verification of your degree from your medical school (credentials verification). This has nothing to do with exam application and you will be issued the scheduling permit once the online application is accepted when ECFMG recieves your documents. Once your permit is issued you can book the slot via prometric for step 1 and 2 ck or step 2cs calender for CS.

6. Issue with score withholding and status re-verification - This usually happens to medical students when they apply to the exam and send form 183. When a form 183 is send, unlike the form 186 which is valid for 5 years, ECFMG might send a request for status re-verification from your medical school if more than six months have passed from the date form 183 was accepted (who ECFMG decides to re-verify is completely random). This is done either via the online portal if your school participates in status verification (note that ECFMG web portal for status verification and credential verification are two different things. Status verification is to verify the medical education details of students and graduates, i.e. whether or not you are a student enrolled in or graduate from the particular medical school; while credentials verification is esentially to verify your degree and your school to upload a transcript). If your school is not in online status verification program, ECFMG sends the request for re-verification via postal mail (neither you nor the college is informed when this happens, so unless your college informs you when they get the form or you follow up about this it can be a headache to deal with). Either way, your college has to complete the form, sign it and send it back to ECMFG. ECFMG will release the score only after the re-verification process is complete (if it was requested in the first place)

7. Credentials verification - When you send the degree and form 345 along the other documents for exam application or independently, the credential verification process begins. ECFMG will send your school the copy of your degree you submitted, form 327-A (verification of credentials/education) and request to send/upload your final transcript. This process is lengthy and takes 1.5 to 3 months (possibly more) depending on whether your school participates in the online credentials verification or not. If it does not, the process has to be done by postal mail and takes a while. It is required that your credentials are verified for you to be issued the ECFMG certificate (even if you pass both step 1,2) when you apply to residency in September. You can check the status of the process in OASIS and when it says "Medical Education Credentials Recieved and Verified" you are done with the process (unless ECFMG for some reason decides to re-verify your credentials which is very rare)

8. ECFMG certificate - Once you are done with step 1, CK and CS and your credentials have been verified, you will be sent the physical ECFMG certificate via post to the address in your record in 3-4 weeks.

9. Start residency application - Usually OASIS will allow you to get a ERAS residency token in last week of June when you would be able to access ERAS, send request for waived LoRs to letter writers via letter of recommendation portal, submit supporting documents like transcipt, MSPE, photograph and unwaived LoRs via OASIS.

10. Apply for residency in September through ERAS.

The process is extremely long and nerve racking (as much or sometimes more than the exams). Hopefully, this will give you a fair idea of the entire process and reduce your anxiety!

Hey dsrp. my name on my MSPE and ecfmg record does not match. Do you think it would be a problem? ERAS supporting documents info in ecfmg website says the name of mspe should be full legal name.

Hi dsrp,

You gave a good overview of the whole process. Great.

Does we require MSPE for credential varification process?
Should we sent MSPE to ecfmg as well via mail, or is it we have to upload during residency application via eras?

My oasis reds "credentials complete and varified". Should I send any further documents to ecfmg?

I think dsrp deleted his/her account for some reason. Anyways, I am in the same stage you are. We don't have to do anything for now. Like dsrp said, once we purchase eras token in june/july, we need to upload mspe, transcript and other documents via oasis, which then sends the documents to our eras application.

Hi, my school is listed as accredited by the ECFMG, so I know I can apply. But my school doesn't have many Americans or people wishing to take the USMLE, so how do I know who's authorized by the ECFMG so that I can get his/her signature? I'm in a Chinese school so communication is kind of a problem, any advice is welcome. I'm currently not in school, so I wanted to get advice so that I can start working on it as soon as I get there.

Actually that's not correct. Though the Dean is usually the official ECFMG representative for most medical schools, that is not always the case. An anesthesiologist was the representative for a medical school my friend went to, so I would not assume that anyone in Dean's office can sign the form.
The best thing to do would be to directly speak with the Dean's office and try to find out who the official representative is from the medical school.

during ecfmg certification m stuck with payment procedure because of some bank issues. is it okay to logout and and pay 65$ afterwards? is any specific timelimit to complete this certification payment ?

There is no timeframe. You can pay the fee as per your convenience.

Hi, There has been alot of comments about Gradduation extension but there is no proper answer, especially from those who are done with residency requirements and have expeience from match seassons.

so Please read the senerio and advise all of us who are still in schools.

I will be an non US IMG when i apply to match in future. I am left with 3 months o graduate and it is now that i cam to know about nit and pick of reidency appication and factors for successful match besides step scores.
So IF I INTENTIONALLY LEAVE AN EXAM OF MY FINAL YEAR MARCH 2017 AND IN THIS WAY MY GRADUATION WILL BE DELAYED to MARCH 2018 WITH ONLY ONE EXAM TO PASS.
Now ADVANTAGES I can think of This are as follow:
1) Fresh Graduate when i apply to match of 2020 in september 2019.
2) Will complete USMLE step 2 during school, say in November 2017, by using 8 months (my college doesnt require to come to college for failed students)
3) There will be no mention of Fail in MSPE, only a times of pass will be mention on transcipts that may give clue to extension but not of failure.
3) after passing that one school exam in MARCH 2018, I can go for 4 month Electives from APRIL to JULY 2018 and right after that I will give CS in August 2018.
4) Return to home country in September 2018 and start 12 Month Paid internship along with step 1 prep.
5) After completing step 1 in JULY 2019, internship in AUGUST 2019 and electives, I will apply too match of 2020 in september 2019 and will apear as FRESH or within 2 YEAR limit.

DISADVANTAGE I CAN THINK OF and WANT you guys to answer whether they are correct or not:
1) ECFMG may consider abnormal behavior.
2)CAn affect match success???

Please answer this issue properly so that many students like me can make oursleves best candidates for future matches.
This is extremely important and I need advise from those only who have Match experience or have gone through the process in the past.

Wen should we begin credentials verification process? Before or after steps?

Depends on whether you are a medical student or a graduate. As the post describes, credentials verification starts when you send ECFMG you're degree/diploma. If you are a graduate, then you will be sending the diploma along with your exam application and the verification process begins. If you are a medical student, you can't start the verification process till you graduate and send the diploma to ECFMG.

Hello, I am new here and I just want to make sure that I don't miss anything.

I've already finished (already payed the fee) my application for ecfmg certification, should I expect any forms to be send to my Uni for verification or that's just it?

Secondly, I am still a med student and I haven't been on any clerkship so far. Does it matter if I leave it blank, even though I will attend clinical clerkship later during my studies? Thus, I might delay my graduation, so what should I write into "graduation expected"? (btw it's the same one as "diploma expected" right?)

And lastly, how long after I pay step 1 fee does it take to schedule my step 1 term with prometric suggesting everything goes as planed?

After you've completed the application for ECFMG certification, you can start the online application for the steps via IWA (no need to send anything to the university)

Ya you can leave the clerkship section blank. In the expected date of graduation you can put a rough date you think you will graduate (does not have to be exact as you'll be sending ECFMG you're diploma in the future when you graduate)

It takes 3-4 weeks for ECFMG to process your application (once it's received) and issue the scheduling permit.

Hi Saxo.

I'm an IMG and a graduate, currently preparing for step 1. I have some questions as i'm new to this application process and its all very confusing to me. I have completed the online ecfmg certification and now i'm starting the application for usmle step 1. I have seen several threads but I'm still confused about following.

1. If i get my form 186 signed and stamped by college official, will i have to mail it or college office will do it?

2. What if i get it signed and stamped by college and mail it myself but write college office's return address on the envelope? will it get accepted? because i've read that if college official signs form 186 then college office should be sending your application in sealed envelope and not the applicant.

3. Do i need to get college official seal/stamp on envelope as well?

4. Do i need to attach form 344 and 187? because the thread only states that form 186 and 2 copies of form 345 are to be sent only?

5. What other documents should i attach in addition to my medical degree (2 copies)? i mean transcript, english language certificate and medical student performance evaluation (MSPE)? do i send these too? In form 344 which is a checklist, it states 2 copies of transcript.

6. Last question is a bit silly but i have to ask it to be on the safe side. Should the photographs be against white or blue background? And ideally what dimensions should they be?

It'd be a big help if they're answered.

No, it's up to you.

Yea, that's fine. Though most do it at least 6 months before so they can be sure they get the desired date.

Thank you for taking your time to summarize this list.

I am a medical graduate now (YOG2014). I sent form 186 when I was in med school but I could not make time for it because of other priorities. Now it has passed the 5 year validation time.

Should I do the verification process from scratch, like anybody else who has never done it? Or should I contact an ECFMG representative to renew the validation time?

Did you check the status of your medical education credentials in OASIS? If it reads medical education credentials received and verified (or something along those lines) you don't have to go through the verification process again. You will have to send a form 186 again when you apply for the exam. If the status in OASIS shows your credentials are not verified, you have to send your degree and go through the process again.

Yes, under Credentials information status reads Medical Education Credentials complete and verified.Does it mean now they have approved of my medical diploma?

In order to register for a Step exam, do I have to pay the exam fee before sending them document form 186?

You're done with credentials verification. You don't have to send your degree. You need to complete the online application, pay the fee. You'll be issued form 186 at the end of the online application.

Can I recieve form 186 paying $ 75 registration fee only without paying step 1 fee? Or I have to pay exam fee to be able to get form 186? I am not ready to say my exact exam date so I just want to bring all papers with me to US. Thank you!

Default Quick Question

I was wondering the fee that I paid before getting to form 186 was that the fee for my Step1 and also, once ECFMG clears my form 186 do I have to book my Step1 date immediately or could I wait to book it when I feel I am ready to take it. Also, do I loose that fee if I don't book or it stays there until I book my date for the first time. sorry but I am a bit confused about this.

I would also like an answer to this question. In more clearer english: Can i do all the steps (Getting form 186 signed and certified, paying the fees) BEFORE i choose a 3 month eligibility period? Or do i HAVE TO choose a period before i can get access to form 186?

Answering both the above questions. You need to select a 3 month eligibility period and complete the online application before you are given form 186.
The initial fee that you pay (75$) is for ECFMG certification (everyone has to pay that). It's not the exam fee. Once you complete the online application, you have to pay the exam fee and then you will be given form 186. You need to send form 186 back to ECFMG after completing it. After ECFMG processes your form 186 you will be issued a scheduling permit and then you can book your date. Yes, you can wait to book your exam date after you send the form 186 and get the permit. But you need to keep a close eye on the dates available in the three month period that you selected (you can check the available slots in prometric website once you get the scheduling permit). If you wait long enough that all slots are filled, then you might have to extend the eligibility period by paying a fee and sending ECFMG further forms.

You have to keep looking at the website. Nobody would know (maybe except ECFMG). I'm guessing it will open up in 2-3 months

If your school participates in the online verification process (part of ECFMG web portal) it takes around 1-2 months. If not, it can take 2-4 months depending on how quickly your school sends back the documents to ECFMG.
