Form Getting the ECFMG Forms Ready

Hi i am graduate and was filling form 186,187, 344 and 345 please i need to clear my confusion before i can get forms signed from DEAN (because he very unfriendly to students and he won' entertain me twice)

1)Should the date on forms 186, 187,345 match.

2)My name on Final medical diploma (Dr. Nitin Arya) doesn't match exactly with the ECFMG record,where i filled my name only Nitin Arya(coz their were instruction during online form filling not to use Dr. as prefix to your name).

3)Should i send medical school transcripts along with form 186,187, 344 and 345. or ECFMG would ask from my college for medical school transcript.

4)The number of forms to be sent ECFMG in order is
Form 186(with attached photo on form)
187
344 + 1 photo+2 copies of Final medical diploma
345+ 1 photo+
Medical school transcript.

1.) documents should close to each when it comes to the dates, but I don't think the days have to be exact.

2.) I don't think it should be a problem. "DR" is only a prefix and it's not part of your actual name. But calling ECFMG wouldn't hurt. They're really helpful and they should be able to answer all your questions. But I don't think there will be a problem with the name portion.

The other questions I don't remember because I did all of this in 2010 and I don't remember the specifics.

hey,
1) i think there won't be a problem if the dtes weren't matched,but it is better to submit with the same date.
2) it won't be a problem, even i had some issues regarding my name wen i applied( surname coming first in diploma and last in ecfmg), but they accepted it.
3)it's better to send medical trancript nowitself and ecfmg will ask again from your college though.....
4) there need not be any proper order but the order u prepared will be preferred by most.
