Registration ECFMG Registration Steps

Here are the steps that you have to follow when registering for the first time with ECFMG in order to apply for USMLE.

First of all check if your medical school is listed in IMED, if it's not you should inform your medical school immediately. To check the IMED list go here https://imed.faimer.org/

Before applying it's recommended to read the eligibility requirements available in this web page ECFMG 2011 Information Booklet - Examination Requirements

1- USMLE ID
The first step ever is to obtain a USMLE Identification Number also called USMLE ID or ECFMG ID this is done by going to this web page https://secure2.ecfmg.org/usmleidreq...navigator.aspx
After you enter your information such as name, email, address, etc they will send you an email giving you your ECFMG ID (8 digits number) and a password to access the online services.
Wait one week and if they don't send you the email (check your junk folder also) then call them +1 (215) 386 5900.
This USMLE ID is yours forever and you should record it in a safe place (not on your Kaplan lecture notes!)

2- IWA application
Before logging in to IWA make sure of the following:
  • Your Internet access is live and continuous (electricity or Internet outage may prove disastrous)
  • Your printer is ready and has enough papers.
Using your ECFMG ID and password you can log in to IWA and begin the application process. You can do that by going to this page https://iwa2.ecfmg.org
After logging in you will be asked to fill out online forms (be very careful while filling these) and if for any reason you are stumbled upon a question that you don't know the answer immediately, you can save the application and log out and come back to at a later time. Then you will be asked to print out some forms that you have to fill by hand and then send by mail to ECFMG.

3- Forms
Form 344 is a check list of what documents you have to attach.
Form 345 is medical school release request (basically you are allowing ECFMG to obtain your information from your medical school). Two copies needed of this form.
Form 183 or Form 186 these are forms that let ECFMG make sure that you (your name and a your photo) are in fact existent and that this is your signature and this is your ECFMG ID. These forms may have to be signed by your medical school official and/or by a notary public, for detailed information about these forms please read The Complete Guide to ECFMG Forms

4- Documents
The documents that you will be sending with these forms are
  • Medical school diploma + translated if it is not in English
  • Two passport sized photographs
  • Some other documents might be needed (like for example if you have transferred credits)
5- Send the forms and documents
Now that you collected and filled all these documents and forms you should send them to ECFMG Philadelphia office (address will be given to you in the forms and during the online application).

6- Wait for confirmation
ECFMG will tell you by email and by mail that they have received your forms and documents then they will send forms to your medical school asking them to verify your medical school credentials and also requesting your final medical school transcript (if you are a graduate) from them.

7- ECFMG to Medical School Forms
These forms you do not have access to it and you are not allowed to send them to your school, The ECFMG will send them.
They will send one copy of your 345 to tell the school that you allowed them to ask about you.
They will also send your Diploma (that you initially sent to them see step 4 above) and they'll ask the school to authenticate it.
They will also send a form called (Verification of Medical Education) the questions in that form are these;
  • what was the requirements before you entered the school
  • how many weeks of medical education you completed
  • what degree was awarded (if you are a graduate)
  • asking to verifying the attached diploma
  • dean or other authorized registration officer's signature, date, and name
  • any interruptions or extensions in your medical education
  • any disciplinary probation you had or currently have
  • any professional misconduct that you have during your school years
  • any negative reports about you in the school
  • any limitations or special requirements by you
8- Medical School to ECFMG
Your medical school would have to respond to these forms and should also send your final medical school transcript (in English, if not you will be asked to pay for the translation fees).
The last two steps may take several months! However, if your school participate in the ECFMG Medical School Web Portal then it may take only few days.

9- OASIS
After waiting for steps 7 and 8 above to finish you are now ready to check your status by logging in to OASIS (using your same ECFMG ID and password). The link is this https://secure2.ecfmg.org/emain.asp?app=oasis
When logged in look at the left hand side panel and click on "Medical Education Credentials and Standard ECFMG Certificate" and the status that you should be happy with is this "Medical education credentials received and verified", any other status means that you're not done yet.

10- Apply
Once you received that status you can now apply for any step you like using IWA or OASIS.
The fees for the exam has to be paid at this point. Current fee requirements are listed in this web page ECFMG | Selected Fees
Note that you are applying for an eligibility period not for exact exam date.
The eligibility periods are three months for all exams except CS it's 12 months.

11- Scheduling Permit
When you finish applying online you'll get a scheduling permit with permission confirmation number and exam registration ID that you should print out and take it with you to your exam. (note there is no more orange slips that you have to wait for in the mail as before).

12- Prometric
After receiving a confirmation email and mail from ECFMG for your application status (Scheduling Permit) you can then go to Prometric website and choose your exam date (of course within your eligibility period).

Notes:
  • These steps are general and there might be significant variations depending on your special circumstances.
  • Be very careful while filling any form online or offline as they will held you responsible for any little thing you say and it's very difficult to change thereafter.
  • Processing times varies with the time of the year and on how fast your medical school responds to ECFMG.

Sorry if that was not clear in the post

Form 186 is for graduates
There two options:
  • You can get it signed by Medical School Official (before step 8 above) of course this requires that you are living near your medical school at the time.
  • Or you can get it signed by notary public (after step 8 above has be done and ur medical education credentials approved approved by ECFMG) this is useful if you are living away from your school of graduation.

In any case you are not allowed to apply for the exam (step 9 above) before having Form 186 complete and valid in your ECFMG file. Note this is a new rule and it was not there before July 2009 (See this thread for more details).

thanks sobio for the great information. this process is so long. well today i have filled the application for the ecfmg no. so after a week i will get the number and password, Right?i read all the steps but please guide me step to step, i really scared by this long process. Why printer is needed which you mentioned in 2nd point? I have good internet. now what i have to do now?

i am applying for step 1. and dont know much how to apply?

what is credentials? and how they are transfered?

should i contact my medical school to respond fast to the ecfmg to get the process fast?

Yes, usually it takes less than a week when they send you your new ECFMG number and password.

You need the printer so that you can print out the forms that they'll present to you after filling the online forms. These forms you have to fill manually and send them to ECFMG via regular mail.

Your credentials are your medical school transcript and diploma. Transcript is the one with marks and number of hours ..etc while Diploma usually a letter written in big font with your school letter head and stating your name and your date of graduation.
If they are not in English you have to translate them.
ECFMG will ask you for the Diploma and will ask your school for the transcript.

If your school has a history of delaying responses in the past then YES it's better to call them to expedite and to make sure that they don't make a mistake when replying to ECFMG.

thanks sabio! thanks for answering my each and every question. this forum is superb and have very supporting staff.so i think when i will get my ecfmg number , then i have to fill form 344 then 345 and then 186 as i am a graduate.

Yes, the only way is to verify with your school because ECFMG does not publish the list of schools enrolled in EMSWP "ECFMG Medical School Web Portal".

It's not that so critical, go ahead and do it.
Your computer will not shut off if it is a laptop.
At all points during the registration process you can save your work so that when you log in again you return to the same point.
If your application is suddenly shut off due to Internet disconnection or electricity outage all what happens is you lose what you have entered (and unsaved info) and you can come back again and finish it.

hey, i got my ecfmg number and password so i started the IWA online application for step one. the first question it appeared was my eligibility period. As i was not sure about my eligibility period , i saved and logged out. It was also written on that form that this form is valid for 14 days. Now i just want to know that what will happened if I am unable to complete the form within 14 days because i need some time to decide about the eligibility period. I am worried.

How our medical school credentails are verified bye ecfmg? I mean to ask that what we have to do to get credentials verified? which form we have to fill? or we just have to start the IWA online application? please guide me. how to do this? i have started the online IWA application.

i emailed them all my required info and waited for like 3 weeks but could not get any responce then i called them , a lady reconformed all my identification and send me only ID no. without any password on my email address so what should i do know? how i can get password?

After step 8, when the status at OASIS reads "Medical education credentials received and verified", is there a time limit or anything before one has to apply for the eligibility period? I ask because I plan to take the exam over a year in the future, but it might be a good idea to get this tedious and unpredictably long process over with as soon as possible.

Once, it is validated it will stay valid forever (unless something extraordinary happens, such as your med school sending another wrong/bad documentation or reports).
You are right, work on the validation right now even before setting your eligibility period. It is a piece of mind to have it done with.

awsome info!!!

im an intern right now..i think its a good idea to get all the credentials verification done in time while im still at school doing internship...but the question is: i want to apply for my step1 after my internship is over that is after i graduate..and if i verify the credentials as still a student...would that create any problems?? i want to start the process because my school is not familiar with usmle system although it is listed in imed list.

You cannot verify the credentials while you are still student because you don't have a diploma and transcript yet.

I am in medical school but I do not think I will be able to give the Step 1 exam in the upcoming clinical year. If I do steps 1-9 listed in the original post while I'm still in medschool, but wish to give the exam after I have graduated, will I have to complete any additional forms, or are there any complications?

You'd have to resubmit your forms if your status changes from student to graduate. So if you are not taking the exam while still in medschool then I suggest you postpone your paper work and you graduate.

Would I be right in assuming that even if I were to give the step 1 exam while still in medschool, but did the step 2 exam after graduating, I would have to resubmit my forms as well?

Yes, you'd have to resubmit Form 186, 344, and 345. In addition your school has to verify your transcript and diploma once you graduate.

You are right, work on the validation right now even before setting your eligibility period. It is a piece of mind to have it done with.

According to the new 2011 regulations, you are required to verify your status before you can apply for examination (including applying for an eligibility period).

hey can anybody clear it to me that how long does it take to process a form sent for USMLE exam to the USMLE office?? a friend of mine got her verification mail of recieving the form and obtained exam date in a period of overall 2 months approximately ...is that normal?

so to start this verification process i must go to IWA and get all the forms printed right? form 344, form 345, and form 186?

Yes, you have to finish the verification process before you can apply to an exam. The verification process include the forms you mentioned and yes you are right you can get them from IWA.
As for form 186, it varies. If your school participate in EMSWP then they'll ask you to sign it by notary public only, if not then they'll ask you to sign it by both medical school official and a notary public.
Am not sure, but I think you have to pay at this time before getting verified.

Hi,.. i need a little info..

Do i have to finish the step 7 mentioned in the first post in order to be eligible for the exam..

My Oasis account says

Just applied for Step 2 CS, paid the fee 4 days ago and still unable to schedule my exam. It seems that every time you apply for an exam you need to go through the process of verification again and fill out applications? Is this true?

No. That's not true. There must be something wrong with your status. Maybe they have invalidated your Form 186 or they are waiting for your college verification.

All schools listed by IMED List of Schools can have their graduates register with the ECFMG.
EMSWP is an electronic portal provided by ECFMG for schools to participate if your school does not participate in EMSWP you can still register by papers.

Notary public is someone legally empowered to witness signatures and certify a document's validity and to take depositions. This person usually has an office and take fees to authenticate your papers. All countries that have jurisdiction has notary public offices everywhere in major cities. Try to enter the word notary public in Google and then translate it to your language and am sure you'll immediately know what does it mean.

hi...
my college hasnt received the documents sent by ECFMG for medical education credentials verification..it has been more than 3 months already...i was planning to request ECFMG for resending the documents again...i jus wanted to know if there is any way by which ecfmg can send those documents to my med school by such a service, that i get a tracking number..
please help...

I've got my ECFMG no. and password, so i logged into the site mentioned in step 2 above. after changing my password, it appears "Submit an Application for ECFMG Certification". but when i click on tht, this appears:
"Your session has ended.
To complete the log out process, you must close your browser."
I've repeated it no. of times, and its the same.
I have NO problem with my internet or power
what shud i do??

Here's what you should do;
Log out from your ECFMG account.
Close the browser.
Clear your browser's cache (check your browser's help information on how to do that)
Restart your computer
Log in back to your ECFMG account and things should start working.

sorry bothering ya fellas for multiple posts...
i don't wanna look naive here but i have attached the final two steps in my application

i haven't encountered anything that says that i have to fill in forms and send it although in the 187 form it says that there are the forms 344 and 345
where can i find them?

does this step come after paying the fees or before it?

please let me know as this application will expire withing 14 days

well i save and logout and that was in November 2010 and i didnt pay within 14 days
and after 14 expiration i check my OASIS in ecfmg i find there is only my name and my phone number with no form of certification that they send it to me via mail when i apply for the exam because ididnt pay within 14 day

actually there is no save button. i tried it twice and everytime i logout i have to refill the blanks again ;D

but u will find that u have a saved application, not filled u have to fill it each time in case of termination of the process in da middle...

hey im a lil confused....can some1 help me out...i have already paid my fee n taken a triplet n then i sent out my form 186 to ecfmg that i got certified from a notary in USA...i got a email from them saying they have received my form...its been 3 weeks n still i have not heard from them after that...will they inform that my form has been accepted or will they form after my credentials have been verified...when i check my online status it says form 186 in process...also from what im reading in the previous posts i cant extend my triplet before my credentials are verified so whats happens if they arent and my triplet starts

Wait another week and if they don't tell you that Form 186 is verified give them a call.
There two scenarios: either they'll allow you to extend your eligibility period without waiting for credential verification and in that case you don't have a problem. OR, they will not allow you and in that case you have to give them a call and see what they say about it.

But for myselfs, when i'm at step two: "IWA application" they ask me to pay the fee and i don't get any form to print and send...should i pay and then i'll get the forms later?

It means that if your medical school has given you such documents, you must send them to ECFMG (not the originals, but certified copies).

I want to ask about fees, I located the ECFMG website for CS fees. It is $1355. I also found other info that if payment is insufficient, application will be rejected. Is the $1355 the full final fees ? or there are other fees like processing, application...etc ?
Can I put the money in my Oasis account even before start the application process for the first time ?

Internship is what you do while you are still undergrad. Clerkship usually means the electives that you do outside your college of graduation yet again counting toward your clinical curriculum requirement.
I don't know what they mean by government service.

That's the total fees, there will be no more fees.

Default Reg. Item 5 in IWA application

Hi All,

A complete newbie here. I followed the steps Sabio listed so wonderfully here. While filling out my IWA application I am stuck at item 5. I recently finished my house surgeon course two weeks back. So in item 5

1) if i select student > I confirm that I am a student officially enrolled in medical school > I confirm that my medical school is listed in the International Medical Education Directory (IMED) and that the "Graduation Years" in IMED for my medical school are listed as "Current." (my school is listed as current in IMED but since I graduated this year would it still be valid because by the time the forms come to my college the status might have changed to include this year as graduating)

2) If I select Graduate > I confirm that I have graduated from medical school > I confirm that the medical school from which I graduated is listed in the International Medical Education Directory (IMED) and that my graduation year is included in my medical school's IMED listing. (My graduation year 2011 is not listed on the IMED site)


Any clarification regarding this will be grateful.

It should be your status at the moment. If you are a student then say student, if you are graduate then say graduate.
If you say student, you have to then change your status later on when you get your Diploma and Transcript.

HI dr.aisha,
If you do not have your medical diploma yet, you need to send a letter signed by your dean/ principal stating that - you are a graduate from that college and that you will be receiving your medical diploma certificate at so and so date. (date has to be approximate )
The letter needs to be worded exactly as per the ecfmg requirements. You will find the exact words in the ecfmg information booklet under the topic Medical Education Credentials.
Send this letter along with form no.186 and when you do receive your medical diploma, you can send it at a later date.

hi,
I need to book a CS date in during early november. but im totally clueless.
info about me:
-medical student
-not yet taken/applied for step1 or step2 ck
-completed the online part of the application for cs

Now, as you may all know, Im late! and also confused as to how to go from here.
  1. I filled up form 344, 183, and 187. do I need to fill up 345 also? as I remember reading somewhere that form 345 is for graduates?
  2. My name on my medical records and my name on my ecfmg- For eg, the name on my ecfmg application is Laura Henry Richardson, while on my medical transcripts is Laura H Richardson. Is this okay? or does it mean that the name on my transcript does not exactly match the one in my usmle application?
Please help!

Hello
Im very new to this whole process. I have read your article and i really liked it because given the fact its my first time i got alot of things cleared. But im confused in some points.
Do we have to pay to get our documents verifed by ECFMG? Because what i have seen is that they wont generate form 186 unless you have paid them the required fees for the exam or is it that we should get form 187,344 and 345; fill it up and send it to them with our documents.

And that should complete the process of Credentials verification and i can pay the fees for the exam when i m ready for it. Did i get it right or is there something i missed?

im preparing for my final year exams but want to get the tedious process of verification done beforehand.
