If your name in ECFMG record and name listed on your med diploma don't match EXACTLY, then you must submit attested copy/copies of any one or more of these-

  • Passport (including the pages with your photograph and the expiration date)
  • Birth certificate
  • Marriage certificate
  • Official court order
  • U.S. Resident Alien Card
  • U.S. Naturalization Certificate
OR
letter from your Dean, Vice Dean, or Registrar of ur med school

I had a similar problem.I got a letter from my med school dean that verified that both names belong to one and the same person.

READ ECFMG info booklet -Under 'Your ECFMG Record' there is a section related to Changing or Verifying Your Name.

Not sure if I can post a link to the exact page on ECFMG site.

Some universities frm India write last name-first name-middle name.Acc to ECFMG,the sequence of names on ECFMG record and diploma should not change. They should submit documents for verification of name.

Hope this helps u, Ravikanth. Hope its not too late.

Default Does spacing in my lastname matter

Hello
Reading this tread I think I got some ideas but I ask my question anyways as it really worries me.. In my passport and all other translated documents(they translate them all based on passport) the 2 parts of my lastname are written without a space(alipourfetrati) but when registering I didn't notice this so I certified as "alipour fetrati" which is the correct form in farsi but as I said translation will be based on passport
so should I use form 182 and change this? Would the space matter? How long would it take for change to happen?
