Form ECFMG Form 180

Hello,

I have finally completed my USMLE registration process and will be taking the exam in December. The next step for me will be applying for ECFMG Identification Card ( as I don't have an English photo ID). I understand that to get the ECFMG ID card i need to use the form 180 to apply for one.

If somebody has relevant experience, help will be greatly appreciated. Form 180 is not available on the website so apparently I need to contact the ECFMG to request the form.

*What would be the best way of doing that? Should I fax them or mail them ?
*What should I write in the request? Is there a specific form or do I just state that I am in need of form 180?
*If you have done this before could you please tell me how long it will be between sending the request and receiving the form?
*Will I receive the form in the mail or by fax?
* How long between sending me sending the form filled out and receiving the ID card?

The usual way is by mailing them a request.

But you can also call them and ask the person on the phone to accept your fax, you have to know the name of the person in order to mention it in the Attn: field of your fax.

It takes about 3 weeks.

They send you a mail, not a fax.

Ooops, sorry I don't think you can use the fax.

Because they will want a photograph, so you have to mail it.
