Yes it is valid.
But in such case it must be sent to ECFMG ,for verification, addressed from your medical school(and not by you).

Notary is needed when you yourself want to send the identification form, without involving your school.

Thanks jasmine

Just another quick question. While sending the form 186 to the Uni, do we also accompany other documents such as forms 344 and 345 along with diplomas and pic in order to forward all this in one enveloped from the Uni. Or this additional documents can be send to the ECFMG by us?

Yes the other documents should be sent along with the identification form, from your medical school only. Do not send them from your side.

All these documents/forms have to be sent by your medical school only. (The choice is given only for the identification form which could be sent either by your or your University).
So it would be wise to accompany these documents(i.e. form 344,345,diploma etc.) along with the identification form in a single envelope, if it is to be sent by your school.

Default Embassy Signature

Hi everone,
I m staudying in Bulgaria and today I went to the Dean of the university to ask about the person responsible for the form 186 signature. I have finished the online part of the application, printed the form in the right dimensions and put my photo on it etc. Now all I need is to get the form signed.
So walked in the office with a couple of friends who are going through the same process and we did not expect what came our way (foolishly thought everything would goo according to plan).
The 'rector' (person close to the Dean) told us that the university will basically refuse to sign and put a stap on any kind of form that they don't know about. I tried to explain for a good 30 mins what the whole thing is about (Why this form needs to be signed, that the person respoinsible for it will know what it is, what the entire process is like, even what the usmle essentially is and much more) Ofc the guy refused to listen to me and kept saying that he doesnt care why I need this or anything else and told me that the only officially signed form I can get is one specific paper printed and signed by the deans office stating that I am a student of this uni.
After I tried to explain that since the University is listed by the ECFMG as one that can provide this signature on the form he started making fun of me asking how long I have been working at this uni, as I seem to know so much... this was it! I gave up trying to explain as he only wanted to scare us away and obviously wouldn't help at all!
Went to the deans office, asking to right an official letter to the Dean asking him if he knows who the person responsible is. The secretary recognized the form (186) and told me that she knows what to do... I trusted her till she basically told me that she ll give me the same form that the previous guy was talking about.
So tmr I m gonna have that form signed by the university verifying that I am a student of this uni but still they are refusing to sign any other kind of form.

My friend found out that it's actually possible in this case to go to the american embassy and get the form signed there!
Hopefully all will be okay and the people there will know what to do!

Any of you guys know whether we need to bring a notary with us? Or will there be one there?

Any help would be greatly appreciated!!!

The process varies if you are a medical student still or have graduated. If you are still a medical student, there is no other option other than form 186 being signed by an authorized official at your medical school. Have there not been any others who have been through the process before you? Try to talk to the dean perhaps, maybe he would have a better idea or try to get your parents involved as dealing with unhelpful administration can be a real pain in the ass (trust me I know!). If there really is no authorized official appointed by ecfmg at your medical school yet (call up ecfmg once and try to find if an official from your university is appointed or not), then your medical school has to get in touch with ecmfg by sending them a letter that so and so person would be the official representative for ecfmg from this university. (Ecfmg will guide you through the process in detail if u call them)
If you have graduated and recieved the diploma, then its a little easier as the form 186 can be signed by a notary and you can then send the form along with the diploma and other documents by yourself.
Once your form 186 is valid and your application is accepted, you will be issued your scheduling permit.
Irrespective of what you do, when ecfmg recieves your diploma they will send a request for verification of medical diploma and request your university to send your transcript (its a lot easier if your medical school participates in ecfmg web portal for credentials verification; otherwise this can take anywhere from 2-5 months)

That sucks my friend. I don't think american embassy can help , though you can give it a try. Unless you have graduated, form 186 has to be signed by authorized official. If there has not been anyone before, like I have said call up ecmfg and find out in detail what you have to do. Get someone, parents or mentors or professors who have a good impression on you, to talk with the dean and ask him to send ecfmg a letter of university head that he would be the official representative. Say you will pay for the courier and everything. I know how these people in administration are, they don't give a **** about students; so you will need someone they are willing to listen (parents, relatives, etc)
