Form Resending form 186 for step 2. Are any other documents required?

I had filled in the form 186 for my step 1 and it was accepted.

However, I realized at the time of seeing my scheduling permit that my first and last name were misplaced on the form, that is singh vienna instead of vienna singh, so I had to send in a name change request form to the ECFMG to get my name corrected.

I took the step 1 on the same form 186 however it meant that the form 186 on my file has become invalid and I have to resend it. I have since changed countries and I have to get the new form signed both by notary and medical school.

The other forms (344, 345, transcript) I sent at the time of step 1 are in the process of verification, my medical college has verified and sent them back and ECFMG has received them.

I have got the new form signed by the notary and am sending it to the college.

My question is am I supposed to send in any other document besides my form 186? do I have to fill in form 187 or any of the others to send back to my college?

For the step 1 my 186 was rejected thrice due to some reason or the other, so I want to be totally sure before resending it to the college for verification.

Please do advise me

No you don't need to send any other forms at this time. It's just your Form 186 and wait for the credentials verification by ECFMG.
