Registration Item 8. U.S. Social Security Number and/or National Identification Number

I started the Registration process for USMLE exams as a student...I am dual citizen, U.S citizen & other citizenship.
I filled the U.S social Security Number, But I don't know what to do for the other 2 question related to National Identification Number; they asks about the Country & Number.

We don't have national ID number, I have a passport that has a number.
The problem is that my name in the Passport that contains the number is a bit different than my name in US citizenship (ECFMG records name matches US citizenship name).

IWA application instruction :
Item 8. U.S. Social Security Number and/or National Identification Number
If you have a U.S. Social Security Number, enter it in the space provided. If you have been issued a national identification number from a country other than the United States, enter it in the space provided and specify the country that issued it.

I saved the online application & logged out.
so what do to do ?

Stop You don't need the national ID if you are US citizen

If you are going to take the exams and continue the ECFMG registration as a US citizen then you don't need to enter a National ID number.

After I finished the application process. I got form 186.
When I printed it:
Header: it shows ECFMG & page 1 of 1
Footer: it shows the URL & date.

Is this acceptable ? or I have to modify the printer to not show this ??

so what do to do ?
I also have dual nationality. I have registered myself in the ECFMG As a us citizen.
I have the same question you had and I have seen the reply that the other user gave to you, he told you not to fill in the space that ask you for the other national Id number, just the one of the social security number.
The thing is that in my country we do have an Id that we use for voting and other issues, in this Id and in my foreign passport it appears my name that slightly differs from my Us name. Should I use the number that appears in this Id or in my foreign passport? Or should I just omit this space like you did? I ask you this because o have another problem with this online application, in item 13, Citizenship, which is: Select the country of your citizenship at birth, upon entering medical school, and now. My question is what should I answer? Because I was born as a Us citizen but I entered medical school as my other nationality but since I have dual nationality I never rejected the Us nationality, so which nationality should I use in this three spaces?
I would be very greatful if you could help me with this! Thank you.
