Help Problem with Last Name

Hi,

I have problem with the arrangement of my name

In my passport and School ID the arrangement is recorded as "Tan Hui Jack Peter" with "Tan" being my surname/Last name.

However, when I followed the instructions... the final arrangement of my name on the ECFMG/ USMLE website was "Hui Jack Peter Tan"

This has caused much worry and was wondering whether I have to change the name on my application by sending a form to ECFMG. Or are there alternative solutions.

If your passport, Diploma, and Final Medical School Transcript indicates that "Tan" is your last name (even though it's listed first) then you don't have a problem.

If they are not, then you need to contact the ECFMG and let them know. They'll probably ask you to send a documentary proof of your first name - last name from your medical school.

so i just need to show evidence that "Tan" is my last name.
