Default About faulty test questions.

I'm not much familiar with the USMLE exam regarding faulty test questions and wonder what the policy is if there is any.
My questions are
1- Has there been any test questions cancelled because of being faulty and considered out of evaluation after an USMLE exam has been performed.
2- What is the policy if an attendee thinks a question in the exam is faulty, can he/she take any action agaist the USMLE board.
3- If opposition is allowed to test questions where can I find the rules and guidelines about the steps an attendee can take.
4- Since medicine is not like mathematics sometimes you can find two opposing data about a subject from different eligible resources. Does USMLE have a reference resource for minimizing conflict about the test questions, if so what would those reference resources be.
