Question Straight to fellowship after european residency - is it possible?

Hi!

I am a 32yo MD (2004), PhD, a good impact factor publication track, and now doing 5y residency in Allergy/Clinical Immunology in Europe.

After completing residency I would be very much interested in a 2-3year fellowship in a good US research university.

What are my options?

1. Can i just skip US Residency and apply for fellowship?
I really dont want to do 3 more years residency.. is it really necessary to get fellowships?

2. I understand that to practice in the US I need USMLE 1, 2, 3, but then which "board"/"comission" is responsible for validating foreign residencies?

3. Let's say I did not get into any fellowship program, could I practice as an Allergist after USMLE3? Can i apply for Allergy/Clinical Immunology Board examinations and get certified straight after USMLEs?

4. Are fellowship salaries good enough for a decent life-style in the US?

Thank you for your help!

Nobody can practice medicine in USA without doing residency. If you want to practice after fellowship you have to go back and do the residency.

You can do fellowship if that fellowship did not include surgical procedures, to know the details you have to contact fellowship programs directly because each program has different rules.

There is no board or committee that will validate your European residency because USA does not recognize any overseas residencies. The ECFMG is the institute that you should be contacting for your USMLE exams and the FSMB later on for your Step 3 exam.

Fellowship salaries range between 45K to 60K per year, descent enough specially in cheap states.

Thank you for the info Steptaker. That residency requirement makes it really unattractive for an european specialist... After long training in Europe I would have to take 5 more years only to do what I can already do in any EU country

I have a question about this line below

If that is the case, then what is the use of USMLE? I know Wikipedia cannot be a trusted source but it is a quick reference for me. On Wikipedia, it says that

The United States Medical Licensing Examination (USMLE) is a multi-part professional exam sponsored by the Federation of State Medical Boards (FSMB) and the National Board of Medical Examiners (NBME). Physicians with an M.D. degree are required to pass this examination before being permitted to practice medicine in the United States of America

So it seems that USMLE is simply a stepping stone to get IMGs eligible for USA residency programs? I am a little confused at this point.

I am afraid practice medicine in this case means practice under supervision - at residency position.

Though fellowship is not an independent practice. Can an international physician (who finished specialty) with USMLEs do fellowship?

Hi everyone...
I know this post is old, but I'd like to reply.

It is possible to practice in the US just with the fellowship. I know 3 (European) MDs (oncologists) that, after completion of residency in EU, came to the US, took the steps and applyed for fellowship.
After fellowship all 3 were hired by the same hospital. There are certain institution that would hire you without residency, but just with fellowship.

The only difference is that you are not board certified, and that you can't practice your specialty wherever you want.

Of course you can. In the same field of your specialty.
