Default problem in passport name and certificates

hi i have a problem in filling my 186 form as my name in the passport and all my certificates aren't the same. my passport has a full abbreviated surname ,whereas in all my certificates its just the initial of my surname. so heard that a letter from the college dean would do . guys does anyone have the letter format which ecfmg prefers.and also is there anything else to do apart from a letter from dean.? plz plz do help me .. as my exam is getting delayed due to this.

Hi Generaly I've a problem with my birth certificate that my d.o.b is 28/05/1991 but unfortunately I changed my d.o.b as 28/05/1992 in ssc certfct,nw it became a problem to apply for passport.
