Help An IMG with three queries

I am an IMG.

I have three queries:

1) MSPE/medical school transcript/non waived LoRs need to me sent by the applicant to ECFMG at time of applying for residency. I mean the applicant needs o send it together to ECFMG for uploading in eras (not the medical school) Since I would be in USA at time of applying its no concern if i send the documents from USA itself.
2) On DSF form we do not mention about the waived LoRs but only the un-waived LoRs. which the applicant herself sends,,,
3) Lastly if ur applying late after your graduation ends u need to fill up the gaps in ur cv,,,i mean is it important do they ask or are concerned about what u did each and every month from date of your graduation,,,

There's no problem. You can send them from a US address.

The DSF can be used for both waived and non-waived LoRs.
For waived LoR, the letter writer send the DSF.
For non-waived LoR, the applicant send the DSF.

Yes. Programs directors are concerned about CV gaps. Usually six months of a gap is acceptable but more than that they'll start to question your eligibility.
