Default Foreign MD/PhD student (currently 1st year of PhD) - when to take Step 1?

Hi all,
I'm on an MD/PhD programme (not in the US) and have recently started my PhD, after completing pre-clinical medicine, and some clinical rotations. I have ~5 years until I graduate properly with both degrees.

I am currently deciding between studying for Step 1 during evenings/weeks around my 1st year PhD work, or waiting until I finish my PhD and go into my penultimate year of medicine and studying alongside studying for clinical exams at medical school.

Anyone had any experience studying for Step 1 alongside a PhD?
