Default European IMG with a lot of questions

Hey!

So last year I f-ed up second year of medical school for all kinds of reason (not going into detail), and I want to use this to rise from the dead and do something great with the chance I've been given to study medecine and give my entire life to it. For some crazy reason, this idea of studying in the US popped up in my head. Why not? But I quickly came to understand that it's not that simple. So I have a couple of questions.

1) I read you needed to do rotations/observations IN THE AMERICAN SYSTEM. Can that be done in American hospitals outside of the US? And do I just randomly contact that hospital explaining my motivations etc? Do these have to be done in agreement with your own university, or are these observations independent?

2) Where I came from, there is no concept of student debt, but if I had to pass those USMLE exams, that would absolutely mean asking for a student loan that could go up to 25.000 euros. Is it really worth the risk? Cause I feel like if I fail, I will be in a lot of debt before even beginning my local residency.

3) I read research is a good thing to have on your "cv". Does it have to be research in America? Can it be research at your own campus?

4) What kind of extra-currical activities do you recommend?

5) I was wondering if it would be better to to my residency here and then do a fellowship. Do you think it's a good idea? What involves that proces?

6) Are TOEFL and/or IELTS still required once you pass those USMLE-exams?

7) Is there someone from Western Europe or any other country for that matter that is in the same situation (early in his medical school) and wants to plan and think about it with me?

8) Are there coaches that help you with deadlines etc?
