Hello , First of all Congratulations on completing your fellowship in IM in India , Its a huge accomplishment , believe me i know how tough it is , as i am also doing my IM residency in Pakistan and aiming for U.S residency in IM .
Ok so you appeared in step 1 in 2005 and got 88. well to be very honest your score is slightly below average , and lets say you get high 90 ( 225) in step 2. But still you have to get USCE ( Hands on U.S clinical experience ) to become competitive. What i suggest that you should do a 2nd fellowship in your own country , like in gastro , cardio , Endocrine , etc etc . I dnt want to shatter your hopes but getting into U.S residency system is getting tougher by each passing year . Than again the final decision is yours .

you started USMLE 6 years ago , and as far as I know ,but actually I'm not so sure bout it , that you are supposed to finish it within 7 years , that gives you only one year left to get it done , so if you are gonna pursue residency then I think you should get started asap.
just thought I'd bring this point to your attention.Good luck.

If you did IM residency back home then you have a good chance. make sure this time you score above 90 in step 2, you may apply to this match. make sure you apply to IMG friendly programs.
goodluck
