LOR from Russia suggestion needed

Hi all,
I need your adviсes...
7 years ago I have been in clinical/research work (in ped orthopedic), I made thesis (dissertation) and in 2007 became PhD. Then in 2009 year my (former) Pediatric Orthopedic Science Research institure asked my rights for publish my dissertation (mostly without changes) as monography. And in 2010 it was published in Saint-Petersburg (of course they included my name as author too). Couple days ago I talked with my former science supervisor (about send me author's set books) and btw I said that I in Canada and about my planes (residency etc)... actually he was little bit shocked :eek: :)
And he said that if it need that he can write me LOR.

At the first I would said that in Russia LOR is not popular. For example if I saw LOR from a candidate (I worked as med chief) I know that it was written by friend of candidate so I can not rely on it. So why I have not any LOR absolutely.
Actually I very confused... I know that LOR should be "fresh", and this my LOR - will be very old if my science supervisor write it with old date but in the other hand may be old LOR better than absent of LOR...
Other problem: my supervisor can write it by himself only in Rus. Is it better that he will write it in Rus and I make official translation or it is better if he will write it with translator in Eng. And if somebody will contact my supervisor - he can not speak English (Rus and German only), but the Institute has officially translators, need he notes a phone of this division?
I very need your advices.

Need I this LOR or not?
What about dates? May be he can write at least date 2010 when the monography was published?
Can he write the LOR by current date with start "Dear program director" I worked with this doctor...?
My supervisor is orthopedic surgeon, but I will apply in Family medicine or Pediatric residency - how can this "orthopedic LOR" be useful? :notsure:
