From first aid for the match:

"In the Couples Match, the NRMP allows any two people to be matched with residency programs in the same geographic area if they so desire. Any two people can apply as a couple. Partners apply and interview separately at programs in the same geographic region. They then submit an ROL of pairs of programs in the order in which they would accept offers. Because couples are often limited by geographic constraints, they frequently submit more applications to maximize the likelihood of achieving a successful match. To help matters, the ROL gives a couple the option of seeking matches in separate locations or allowing one partner to go unmatched
in the event that a Couples Match is not possible.
Some residency directors and deans believe that many couples do better together in the Match than they would if they applied and matched separately. Couples tend to be viewed as more stable and less likely to leave residency programs; on the other hand, couples matching in the same specialty may prove to be more difficult than matching in different specialties. Given the hassles involved in moving again after residency, especially with kids, couples are also regarded as more likely to contribute to the faculty pool of the institution in which they trained. Nevertheless,
the results of the Couples Match can be unpredictable, especially when
both people are applying to competitive programs. In addition, couples tend to
seek reassurance from their interviewers that everything will turn out well and may thus be particularly prone to misinterpret encouragement as a guarantee about the match result. For more information about the Couples Match, consult the section on “Special Cases” in your NRMP Handbook for Students or consult the NRMP Web site "
