Although many may argue and I respect their opinion, but in my experience in the past few years, in most of the times these criteria posted on the programs websites are just to reduce the number of applicants. I have many applicants with low scores matched to such programs, some of these applicants have other positive factors in their XCV as previous residency, USCE.....
These critieria may be real criteria in university hospitals especially which contain some AMGs. In general, to get the highest number of interviews you need to apply widely to as much programs you can afford regardless critieria.

All USCE are IM USCE ... 1 month externship with an US doc in teaching hospital and general hospital, 1 month teaching hospital observer ship and 3 months externship in private clinic and 3 months observer ship in private clinic and 6 months of volunteer work in a hospital , 3 US LoR ..... 86,96,pass ( all 1st attempt), ECFMG certified ... .... .... I am applying 135 IM program around US in september 1 , most are comm or comm aff .... few 8 to 10 uni program .... Now few program require 87 or 88 or 90 or 93 or 95 in both step ... and few prefer ( not must have ) YOG 3 ..... i counted total 15 programs like that where 1 criteria dont match with me ... but most of these program prefer GC holders and prefer USCE ....
