Do ACLS, the American Heart Association, not the European.

Also, try to participate in research.

Also, do online CME.

I know your situation.. try to take the ALS as late as possible.. Focus on applying for the match as soon as possible, because your deploma is gonna be issued in a date that is a year and a half later than what's written in it (they will write Nov. 2010 while they will only hand it to you in May 2012!!) So you are already gonna be 2 years postgraduation if you applied as soon as can ever be!!
Try to get into clinical research (try Dr Hussain Khairy) .. Focus on getting recommendation letters during internship, because everybody will forget you even existed after that.. And try to do some volunteer work..
Most importantly try to get externship (I think it's possible during internship) or observership.. you can take the CS right after you do it.. If you can't find any in the US, I think it's easier for you in Canada, and it counts..
Try to get as high scores as possible.. For both steps USMLE world is a must, and try to get the online UW, the books printed are not complete.. for step 1 First air is a must read too.. and in my opinion, get step 1 before CK, you will find many questions in CK that are only mentioned in step 1..
Lastly.. connections, connections, connections.. in Egypt and in the US if possible.. Email Universities and hospitals, research centers... Go to the US!!
It's a long road and each step might bring you down if you do it wrong..
Other stuff are not that imp. e.g the ALS .. they are good, but not that important..
These are the mistakes that I went through.. Good luck
