Hi,
I am an IMG from India and want a surgical residency spot.

Can I do my residency in my home country and apply for fellowship in US later, after giving my steps?

Or do I need to do residency in US also for fellowship application?

and what about step 3? Is it necessary for fellowship application?

You must go through a US residency in order to apply for a fellowship position. In order to apply to residency you must be ECFMG certified and have completed Step 1, 2 CK, and CS. You can wait to take Step 3 until you are in a residency program but this must be completed before applying to a fellowship.

so fellowship without residency is not possible? anywhere in US?

For general surgery you only need to go through a residency position. If you wish to pursue a sub-specialty, such as cardiothoracic surgery, then you must apply to a fellowship following residency. For this type of fellowship you must complete a residency prior to applying.

There are research fellowship positions you can obtain prior to residency positions. You would work as a research assistant under a supervisor at a university-based program which could help your application for residency. You do not have to complete step 3 to obtain one of these research fellowship positions.

Yes. You can definitely do fellowships without residency in the US.

I know a friend of mine who did her MS General Surgery in India and went on to do a hepatobiliary transplant fellowship at Baylor, Houston after she completed her USMLE steps.

Research more about fellowships and their requirements. There are plenty of unfilled surgery fellowships every year many of which will accept foreign trained candidates. Most programs need Step 3. Some require only Step1, CK and CS with ECFMG certificate.

True, but if you want to stay and practice in the US after fellowship you need to have completed an ACGME approved residency, in order to get a license to practice
