Nobody can predict the future, but if you want to get into the US now would be the best time.

Take your USMLE and see what you score. This conversation becomes moot if you don't have the score to get through the filters for Anesthesiology. We can't give you a "What are my chances," without you having your scores. Most programs WON'T post on their websites like IM sites do, but they totally filter.

Ideally you need to score >240 on all first attempt passes(on the interview trail I heard this echo'd to other applicants by some PD for FMG). Not all of us had that high though, but you'll get a fair amount of interviews (even some decent places) if you're in the 230s(at least from talking to other applicants on Anesthesia trail).

Even if Anesthesia becomes less competitive in the future, like what's happening in Radiology, I wouldn't expect them to lower score thresholds. Anesthesiology still has a fair amount of in service exams and difficult boards. Therefore score thresholds won't lower even if the specialty becomes less competitive to match to.

I know my program told us (not posted) their cutoff was 225 and first attempt for interview invite. Having the score doesn't necessarily get you there though as you'll see plenty of people that had great scores and not that many interviews or didn't match. I was lucky and got 9 Anesthesia interviews.

I felt Anesthesiology programs were looking for regional ties in a fair amount of places. Most Anesthesia away rotations/observerships will usually land you at least an interview (n =6 ), but of those that did aways, none of us landed at spots where we did aways. I didn't do an away where I landed, nor did I have any regional ties besides. So you can draw your own conclusion from that.

Lastly, attend the ASA. It's usually the first day of the ASA in the evening. There's a meet & greet program directors and residents of program event. You'll pick up at least a few interviews, if not at least get a few more programs to look at your application that wouldn't have before.

I matched Anesthesia. You asked for advice for anesthesia and I added more anesthesia specifics above.
