Warning! Will There Be a Residency Slot for You? - 2013 Match

Not to be alarmist -- but, with the Supercommittee deadline next week, there is a very real possibility that the number of residency positions available nationally could be cut significantly. As you may recall, back in August, Congress passed the Budget Control Act of 2011 to avert the debt ceiling crisis. This Act created the Joint Select Committee on Deficit Reduction, or the "Supercommittee," comprised of twelve members of Congress - six Democrats, six Republicans. The Supercommittee is charged with developing a set of recommendations to cut $1.5 trillion over the next 10 years either through spending cuts or raising revenue. These recommendations are due today, November 23. Congress then has until December 23 to pass these recommendations. If Congress fails to meet this deadline, there is an automatic trigger in place which will result in sequestration, or across-the-board cuts totalling $1.2 trillion. In a sequestration scenario, half of the cuts will come from defense, half will come from other spending.
Why does this matter? The vast majority of funding for residency training in the United States comes from Medicare. Each year, Medicare spends approximately $10 billion to underwrite the cost of Graduate Medical Education (GME). Last year, the President's Deficit Commission recommended cutting GME by approx 50%. Luckily, that recommendation went nowhere... until now. Although the Supercommittee's negotiations have not been public, there is pretty widespread speculation that the Supercommittee may recommend up to a 60% cut in Medicare for GME. According to a survey of program directors by the ACGME, this will results in a very real decrease in the number of residency positions available in the 2013 Match.

I know I just read the article. How much truth is there to it? Are the chances of getting residency in 2013 and onwards grim?

Every year there are tons of speculations of what will happen next year. Since 2006, I've been hearing and reading about impending doom to IMG matching.

While it's true that IMG matching percentages is declining, it's difficult to say that there has been a sudden major dip in the curve from one year to another.

25% of US doctors are IMGs. If you suddenly stop hiring 25% of your workforce, that would definitely result in a health care crisis.

I personally think that GME offices will find a way to finance their programs even if they lose federal funding though I highly doubt that.

5-6 published articles indicate they are cutting the budget, Program directors here in Chicago have confirmed me that next year they will pick 30% less.

again is excellent to be positive, but is great to be realistic.
