Other than taking step 3, having a good reason for what happened on your first step 2cs and trying to get even more USCE, I don't think there is much you can do. You graduated in 2008 but were working in the cardiac ICU and working on your PhD. You have research, publications, LORs, USCE, and no visa issue. The only thing holding you back in my opinion is that step 2cs, but other than that you look decent. Just make sure you are doing something between now and next year. Good luck

Thank you for your constructive comments.

Would emailing/contacting PDs and explaining them the reason why I failed in CS can help me passing the CS attempt filter ? or that won't be helpful?

I don't think calling them will help much. The only way to get past the filter is to have an advocate in the program (connection). It doesn't have to be the PD, maybe an attending, resident or program coordinator. You can only make these by family or networking.

If I were you, I would try to write the reason somewhere on your application (doesn't have to be just in the personal statement).
