Default Should I apply preliminary/transitional

I am applying for anesthesiology residency this year, mostly advanced positions.

should I also apply for preliminary/transitional? not for the backup. I'm not fully familiar with what happens when you match in advance position.

Here's a breakdown of the main program tracks for you:

Preliminary - 1 year program
Categorical - 3 year program (enough to apply for the boards)
Advanced - 2 year program that follows a preliminary

You should not be applying to Advanced positions if you have not completed a Preliminary track program. You should focus on either Preliminary or Categorical positions.

Thank you for your reply, I appreciate your help.

to my understanding anesthesiology residency is either

Categorical: 4 years of training, where training hospital itself offers PGY1 after which you continue on with CA 1, 2 and 3
Advanced is 3 years, starting from CA1 in which case hospital doesn't offer PGY1 and you should finish preliminary year somewhere else first.

what if I match in advanced postion and than start looking for preliminary spot or is it only for those already done with preliminary?

and why does some institutions offer both advanced and categorical, can you apply for both, maximizing your chances?

From my experience, if you were to Match into an Advanced position without a Preliminary year under your belt, you would put yourself in a very sticky situation.

Even if a program offers both Categorical and Advanced, you would still need to apply to each separately and hope to god you can secure a Preliminary position as well.

To be safe, call the Advanced program and see what they require.

But yes, your understanding is correct. Just be careful, if you don't have an internship or prelim position, you may have to forfeit any advanced position extended to you.
