Help IMG clinical clerkship-regarding California Letter

I am an IMG and working on California letter right now. In Form L5, there are 6 rolls to fill in for clinical Subjects. Which 6 Subjects they want us to fill in and what minimum hours for each of them so that it can be accepted? If I have more than 6 Subjects, should I add an attachment?

I contacted the mbc and I had the same query
this was their reply

Once the Board receives your application and the appropriate fees. The Board will mail you official fingerprint cards. You must have the fingerprinting done by local law enforcement and/or Embassy.

You will need to use as many L5 forms as necessary to document all of your training.

as i was posted from my second year onwards I would probably need 8 such forms to give my detailed clerkship postings
