you can submit your application, then you will receive a letter with the missing document , Once you have your result and ECFMG certificate just send them to the MBC

I have a question about the PTAL: it requires 6 weeks of family medicine. I'm a foreign grad and I didn't get 6 weeks (only 4) at my medschool, but I can get my medschool to show that on the L5 form, but they can't make official transcripts with that change. So, if they compare the L5 form and the transcripts, they might spot the inconsistency. Anyone know if this has been an issue in the past, or if they are that meticulous, and if so, what the consequences are?

I received my PTAL yesterday! finally!

I applied for it in november 2013. Was quick to send the documents i had to send and the documents my medical school had to send. Its not that difficult to get it if you are already ecfmg certified,have SSN and if you are quick to send all your documents Its just a lot of paperwork you need to do.

I hope all the work i did to obtain this letter is worth it!

Default questions regarding Ptal!

hey guys!i am so confused regarding this PTAL letter!i have a couple of questions-:
1)i applied online and paid the fees.Apart from the forms i submitter online do i need to submit only the form L1E by mail(notarized)?
2)how long does it take for them to get in touch and tell me who my analyst is?
3)i did my internship in another medical school so how do i fill the psych requirments?do i fill a separate form L6 for every rotation i did during internship?
4)my original transcript says community medicine.Do i get my med school to attach a separate letter stating that community medicine includes family medicine?
5)my clinical rotations,do i need to mention the number of hrs per week aswell or number of weeks is enough?

Hi! It's my first time here on this forum, i'm confused about PTAL. I couldn't submit my application online because they ask for my Ecfmg certfication which i don't have yet, i have only Step 1 result. What should i do??

Help Is Social Security Number mandatory ?

Iam an non -US IMG and obviously do not have a social security number.Does that mean I can not apply for PTAL?


Another question: If there is a gap of more than 3 months after graduation does that mean they will reject my application ?

Hi liquiddreams,

I had the same question as you. All the research I did on this subject clearly shows that you must have a SSN when they issue your PTAL. This is now an absolute requirement. Sorry to be a downer but here is what I found:

Based on all the research the only possible way to obtain one (without GC etc.) is to get an F1 student visa and take courses at a university/community college, and then find ON CAMPUS employment. This would be a reason to be eligible for an SSN.

If you somehow manage to do this, you could INITIATE the PTAL application now and then give the MBC your SSN once you have obtained it and they will issue the letter.
Please note that you can theoretically apply for a ITIN, which has the same format as an SSN, so the Board won't notice. However, once they find out, they will not issue / revoke your PTAL, so this is not a viable option and will only waste your application fee.

Theoretically, it may be possible to transfer as a PGY2 to CA, but in practice this would be very hard to do.

To sum things up: The only reasonable option to practice in California in my opinion is to do residency somewhere else, then apply for an PTAL (you will have an SSN) and then later apply for fellowship in CA.

Default How to re-validate usmle step 1

Hi, i received following quiery regarding my PTAL. My usmle step 1 examination will expire in less than one year on august 2014, so revalidate your passing score on step 1 and contact ECFMG. I contact the ecfmg and there is no such thing for validation of examination after passing step 3. Please help me in this regard.

Default Need help regarding Family Medicine requirement

Hello everyone,
Awesome post about PTAL!
I did not have any FM hours as there was no FM clerkship available at our med school at the time. However, as a resident in Vietnam, I did a lot of community medicine (one month every year). Can I use form L-6 to fulfill FM requirement?
Also I worked as a medical assistant in Cali, can ambulatory medicine replace that and would this experience help in any way?
Please help!

I am in the same boat here.

One question, will programs offer an interview to and rank applicants like myself, who satisfy ALL the requirements for a PTAL save for the SSN, and can prove this, via a letter from the MBC stating that only the SSN is outstanding.

If the programs are willing to consider applicants like myself, after matching and obtaining the visa (J1 or H1B) and the SSN, I still should be able to obtain the PTAL before commencing residency training.

Is there any chance the programs will consider me still? Or would it be a complete waste of money to apply to California programs? I believe that I would be a pretty competitive applicant otherwise.

Here are my credentials:
I scored 262/264/pass for step 1/CK/CS all at the first attempt. I cleared step 3 with a score of 253 also at the first attempt. My YOG is 2011, but have been involved in clinical practice in Internal Medicine since. I have 5-6 months of hands on US clinical experience with very strong letters of recommendations, have 10 peer-reviewed journal publications (7 published, 3 in press), 1 book chapter, and multiple oral and poster presentations in European and American conferences. I am a foreign IMG who is a native English speaker and require visa sponsorship.

hi..i ll be taking my step one in august.i read in your post that it would be better to enter any of the step scores while applying for ptal.but i have also heard ptal takes a longtime to process.is it too early for me to apply for ptal?

Do you think I'm still on time to apply today (May) for the application in September 2015 (match 2016)?

Do you think that this would increase my possibilities for Radiology?
I have heard that the radiology programs in California are very competitive and I'm not sure if I will get many chances with this, counting that the letter is overall like $600 USD.

Hello fellow people annoyed with paperwork.
So I'm midway through applying for this thing.
  • A Certification Status Report from the Educational Council for Foreign Medical Graduates (ECFMG) is required to verify your certification is valid. The ECFMG must mail the Certification Status Report directly to the Board. An Official Examination and Board Action History Report are needed to verify scores on USMLE Step 1 and Step 2 CK and CS. The Federation of State Medical Boards (FSMB) must submit official examination scores directly to the Board.
So for there 2 forms are these what I need?

Hi, Im an IMG. I already submitted PTAL payment and documents. In breeze website it says my application is pending. How do I get PTAL receipt? or something like receipt of payment? I need it as temporary document for the ERAS application.

Default how to fill L5 in PTAL?

I am an IMG and i applied for PTAL but my application is in pending as there is incorrect/ deficient number of weeks in clinical clerkships (esp in psychiatry and electives).

My doubt is that, they asked me to enter all my rotations from 3rd year to internship. My college gave me internship certificate which has the number of days in each dept. But i dont have any info about the rotations in 3rd and 4th years. Can anybody please, give me the format of the clinical clerkship certificate(3 and 4th years) and number of weeks they do in each dept. Please guide me how to fill the L5? and how to make a supporting document(for L5) to be approved by my medical school and MBC.

Hey guyzz ! I have a question i started filling my ptal application breeze online it asked me if I was ecfmg certified , on the first page itself , if we say yes and move on there is another page asking for issue date all of this is in the initial part of application itself , and Im not certified yet, can some one help about this?

any idea what this means

The Certificate of Clinical Training, Form L5, completed by your medical school has total clock hours reported, therefore, the information provided is unusable. Only clinical clerkships which you participated in hands-on diagnosis or treatment of patients in a clinical setting should be reported. No credit will be granted for lectures, theoretical, laboratory, or didactic hours of instruction. To meet the requirements of Section 2089.5 of the Business and Professions Code, you must have 72 weeks of undergraduate clinical training in specific core subjects and electives. If your medical school is unable to provide detailed information so that we may determine that your undergraduate clinical training meets the minimum requirements of Section 2089.5, you will be required to remedy the deficiency prior to participating in any postgraduate training in California. Please request your medical school to document all clinical time completed during your enrollment in medical school. The information may be provided on a Form L5 or on medical school letterhead, signed by the dean or registrar, and affixed with the medical school seal. It should include the clinical subject area, dates of attendance and total weeks of credit granted.

one correction, They no longer accept the diploma and transcript if you send it to the MBC. it has to be sent by your medical school. I tried sending them my original diploma and copy and they just sent it back saying they won't accept it unless it is directly from my medical school

I am a US-IMG graduating from an all English program from a foreign medical school. But the English program at my medical school is not currently listed as an approved school but the native program is.

My question is will I still be able to apply for PTAL? Since it asks for the language the degree was taught in.

Also I won't be taking my CS until May of 2017, would that be too late to complete PTAL assuming everything else is finished for match in September 2017?

Default How to submit initial online application for PTAL .(without giving false information question 2 and 3 )

I had another query regarding submission of the initial application form for PTAL.
After registering in Breeze for the online application for PTAL. I am required to answer the following questions

1.To meet the examination requirement for a medical license, you must have passed one of the acceptable examinations per Section 1328 of Title 16 California Code of Regulations.

2. To meet the postgraduate training requirement for a medical license, you must have satisfactorily completed at least one year of ACGME or RCPSC accredited postgraduate training if you graduated from a recognized U.S. medical school, or two years of ACGME or RCPSC accredited postgraduate training if you graduated from an international medical school. The ACGME or RCPSC accredited training must include at least four months of postgraduate training in general medicine.

I am a graduate of CMB recognized international medical school but I am yet to take the USMLE( answer for ques 1 ) or begin my residency( ans for ques 2) and I am just beginning the process of application FOR residency. How am I to go about applying for online application for PTAL ,paying the fees and fingerprinting if I do not have the above requirements ?

It would be of great help if you could guide me on this.

Dont you require SSN? How will a FMG get SSN on B1/B2 visa?
