It will be difficult to get into anesthesiology with this score, however I don't advice you to give up. You can still get IM, FM or psych. I know some applicants with less than your score matched into anesthesia but those usually have extensive USCE (2-4 US LORs). I guess your publication can also help you.

It is not easy to get these fellowships without residency in the states, however if you want to apply you must at least be finished with step 3.

why is it difficult if the nrmp outcomes in the match says the average img who matched had a score of 229?

I guess these reports don't differentiate between IMG and AMGs who have more chances.

That's nonsense, you can match with 235. There's obviously a whole lot of factors that go into it but I have a similar step 1, am an IMG, and had 9 anesthesiology interviews. If you read charting outcomes, as an img, I think 8 contiguous ranks gives an 70-80% match chance.
I got in the 260s in step 2, have 2 US LOR's and did 4 weeks as a medical student in the states a long time ago with a GP. I don't think that would be considered particularly impressive from a USCE perspective.
If you do well in step 2 you'll have every chance. U.S letters will most likely help as well.

Good Luck

Did 4 weeks in a family practice clinic in 2004 and had 1 letter from that. Had another one from a medical mission trip doing cleft palate repairs. I think it's hard to say how much each particular aspect of an application helps. If I had no american LOR's perhaps I wouldn't have got any interviews. Also, I'm not sure if imgs are more less competitive depending on their nationality. I'm Australian, whether that is looked upon differently than other nations or not I'm not sure. Perhaps, some of it comes down to the mood of the person selecting interviewees at the time. To me, there seemed to be no consistency or predictability to where I got interviews. I got offers from some really great programs and rejections from programs I didn't think were particularly competitive.
I do think, though, it's unhelpful to say that someone won't match with a step 1 score of 235. That's a solid enough score, and if you smash step 2 and manage to get a letter or two from an American attending and perhaps one from a director of a department in your home country, I see no reason why someone couldn't match in anaesthetics.

You can get fellowship directly in Anesthesia and Radiology if you completed your residency in your own country. Lots of people do it so you will get it as well.

Good Luck!
