Default Required to fill out "Application for Appointment to House Staff" prior IV, is it a trap?

One of the places I'm interviewing at for IM residency is requiring me to complete an "application for appointment to house staff" form. Was wondering if I turn this in during the interview, would I possibly be locked into prematch??

Anybody have any experience with this? If so, how would I handle this situation considering this place probably won't be one of my top choices?

I think this is supposed to be a sample of a resident contract or something like that, and I think you acknowledge that you received it. I don't think its legally binding, at least haven't heard of anything tricky like this happening
