Default Need help with visa issue(j1 for resi and fellowship from different countries)

Guys question for you. It affects my residency decision so posting here. Can one have J1 extension for fellowship after residency by getting second statement of need from a different country than the country from where he got statement of need for residency.
Reason I am asking is because I am currently a permanent resident of one country and I am going to lose permanent residency during (medical) residency in US. Then only country I can go to is my country of origin and cannot go to previous country. Can I get a statement of need for fellowship from my country of origin in that case?

Hello, Vinayak. I have a similar situation as you do. I am a Canadian PR and a Citizen of India. I plan to apply to the NRMP match [match day in MARCH 2018]. My PR expires in DEC 2018. Since your post was dated back in early 2016, I hope you got your answer. If you have, could you answer a few questions for me?
1. For a J1 statement of need [SON], the ECFMG website mentions that if one is in my particular situation, the SON must come from the country of last legal permanent residence. I have been living in India and will be doing so at the time of applying for the visa. So which country then should I get the SON from for the J1 visa?
2. If I do have to take SON from Canada, and my PR expires, can I return to Canada and practice?

Would appreciate answers to at least the first question. Thank you for your time.

1. Permanent Residence is where you were primary living for the last 3 out of 5 years. It's the threshold that most organization use however some argue it's based off the last year. Example: If I moved to India for 2 weeks, does not mean I'm not a Canadian Citizen/permanent resident of Canada. Thus where-ever your primary residence is should be used for your SON.
2. SON for a J1 and Immigration Visa to Canada is separate. Completing equivalent residency is require to practice in Canada. Immigration to Canada is a separate issue all together. If your PR expires, you will need to reapply, or most individuals currently renew earlier as they have been in Canada 2/5 years. PR cards are valid for 5 years thus giving you the ability to finish residency and still be able to go back to Canada.
