Default Ortho residents;scored <225 on S1 please advise.

Hi everyone,
My boyfriend is about to start 4th year and has been pursuing Orthopedics since his 1st year. He is terrified he won't match to a decent residency or match at all because is got a 224 on Step 1. He's ranked in the top half of his class, but not top quartile. He has done 3 Ortho research studies, presented 1 in Argentina at the Global Spine Conference. He has gotten high pass on 2 of his core rotations so far, clinical honors but not overall honors on those two because he struggles with test taking. (i.e. The 224 on Step 1).

For those of you who scored lower than a 225 on Step 1, did you match or not match? Did you get interviews at the places you wanted or AIs? If you did match, was it in your top 5? If you didn't match but did a Pre-lim year in Gen Surg, did you get a preferable Ortho spot after?

His top picks are UAB, Colorado, Ole Miss, Jacksonville FL, Gainesville FL, Columbia SC, Vanderbilt, and South Alabama.

He's too busy to post these questions so I'm doing it for him.

As an SO not involved in the process you might not know this, but about 5 IMGs match into ortho every year, so the chances one of them will respond to this is pretty low. Applicants from IMG schools with stellar scores can't even get interviews in orthopedics, let alone match. 224 is probably even below average for IMGs these days, so I think the ortho idea is a pipedream and any further pursuing would only lead to future disappointment and wasted resources.

It's pretty much impossible. Step 1 cutoffs for many programs to even grant an interview for US medical grads is 240 minimum. For an IMG with a low step score there is no chance, regardless of how much research he has.

Another check you can do is look at the ortho residency websites for the schools you listed and see how many IMG's are in the program. I suspect you'll find none.

One quick follow up point. If your SO is in a US medical school there is still a slim chance (probably much to the dismay of people on this forum). He needs to do the following:

1. Score >240 on STEP 2 (this is a must).

2. Schedule 2-3 away rotations in his fourth year and impress the heck out of the faculty. The away rotations are first come first serve and many programs won't grant interviews to people who didn't do away rotations at that program.

3. Talk to the program director at his home school about staying. This will be his best shot.

Other factors that will help offset the low step 1 score ...

Attending a Top 25 medical school.
Honors in Medicine or Surgery.
Strong letters of rec from faculty.
Research (which he already has)

Again this only applies if he is attending a US medical school.
