For IMGs I would say a US LoR from a reputable hospital holds more impact than publications.

However they are both important.

US LoR shows that you are familiarized with the US Health Care system and research publications shows your dedication and scholarly activities.

By the way you can combine both if you do the research in USA, where your research professor can write for you the LoR.
