Default Advice for Next match

I am a IMG from Pakistan and I am looking for some answers.
I applied last year in few programs in IM based on 6 week observership in IM last year and I didnt match. Since November I have been involved in Research in surgery at state university and I have done poster presentation, gonna submit a study and continue to collect data in OR. I didnt get any good opportunity in IM. Now I am in a big confusion. My cv is going towards surgery and it is very tough to get a interview in surgery. If i apply in IM based on cv i have I am not sure i will get any interviews in IM either. I dont know what to do. I still continue to get opportunities in different departments of surgery for research and probably will get LORs from chiefs. Also If some one can tell me about IMG friendly programs in surgery I will appreciate.
BTW my scores are 233 in step1, 239 ck, step3 221

Hello there,

Your scores are average compared to the matched IMG applicant. Thus, the differentiators you need are strong research (# of publications) and strong letters. I wouldn't worry about the field. You can spin it either way. For instance, you can say you're interested in the medical management of complex surgery patients. I agree with you matching in Surgery is very hard for an IMG. you may be stuck with a preliminary - surgery position with no promise you will be selected as a categorical surgical resident. Sometimes, you can use that preliminary surgical spot and demonstrate you can perform well. This can be used to prove you are strong enough to finish internal medicine at a U.S. institution.

In fact, there are some places that slot IMGs to a preliminary - surgery position before giving them a categorical medicine spot. It's not ideal, but it happens.

I recommend this:
--Continue with your research
--Collect the strongest letters you can
--Develop relationships within your research institution including the program director
--Consider applying in both to maximize your chances. I would not share this openly with faculty.
