Star kindly guide me

hi am IMG from INDIA, i discovered this site while preparing for step 1 and would like to say that due to help and guidance offered here stretched my STEP 1 score to 235/84.. I dunno if its a par score or not.
I graduated in 2008 and did 1 year rotating internship after which i got into MD anaesthesia program in INDIA..just completed my program and preparing for step 2.
Its been 4 years since i graduated..will it make a difference on my CV or will y MD be counted as continuing medical education?

I would like to apply for fellowship PAIN / critical care or IM residency
I have 2 research papers (no paper in PAIN OR CRITICAL CARE) and 4 case reports in indexed peer reviewed journals
after giving step 2 and CS should I apply for observerships next year or go for research fellowships?
Basically i wanna know how to go about making my CV appealing to PDs

Your help will be highly appreciated

The average Step 1 score for non-US matching applicants in 2011 was 225. For step 2 the average score was 228.
I think that PD's are more interested in clinical experience rather than research prowess, for obvious reasons.
Your distance from graduation will be mitigated by the fact that you were continuing medical education.
I think you have an interesting CV. Perhaps an observership will give you a familiarity with the American system which you currently may lack.
Just my 2 cents worth, would love to hear other opinions.
