Default LORS as an IMG

Hi all first time poster. Apologies if i'm posting in the wrong place and let me know where to put it if so.

I'm a final year Canadian student in Ireland. Hoping to do Peds this year. Step 1 219; CS first attempt. CK result next week.

I did only one elective in the states (doing it now) and it isn't in Peds. Any letter I get from here would probably take a few weeks to get. Currently I have 4 letters from peds electives in Canada and 1 from my program director in Ireland. What do you guys think I should submit:
1. 3 peds letters from Canada + Program director
2. 4 canadian peds letters
3. 3 peds letters and update my application with US Med-Onc letter

Other info: Canadian citizen looking for J1, 9 pubs no first authors, masters degree, good extra-currics. Any thoughts of my chances with the poor step 1 score? I'm applying very broadly.

Use at least 1 or 2 US letters. It would have been better if the US letter was from Pediatrics. If you decide to use the Med Onc US letter, tell your letter writer to not give you a letter specific for IM and maybe tailor it to Peds in the closing comments. Also, from what I remember the visa process for Canadian citizens is different from other IMGs seeking J1 visa, so might have to research that on sdn or forums (not completely sure about it). Good luck
