Arrow It depends on what you want

The ideal situation if you are looking for both is
3 LoRs FM + 3 LoRs IM ---> Apply for both IM and FM

If you have high scores and your intention is genuinely IM only then yes, convert your FM LoR to IM and apply for IM only but widely.

If your scores are on the low side or if you are an old grad or if your CV is not that astonishing then I suggest you apply to FM "as a backup" because FM residencies are considerably "easier" to get than IM residencies. In that case I suggest you hold on to your FM LoR and try to fortify it with one more and apply for both FM and IM.

Thumbs Up Nice scores

These are fantastic scores and should get you into IM. By the way please vote for Step 1 CK differential Poll. You should choose 10+ to Zero option in that poll.

Now back to our topic.

You did not tell me what's your genuine interest! Is it IM! If the answe is Yes then use your FM LoR to apply for IM. You can either leave it as an FM LoR (IM people wouldn't really mind an IM LoR) or better if you can convert it to an IM LoR (by asking the writer to recommend you for IM programs).

It depends on your graduation year and the presence or absence of USCE and other factors.

However, in general, California IMs are very competitive. You are unlikely to make it into University programs but you have pretty good chance in getting interviews in community programs.

Now, the question is. What do you prefer, Community in California or University in New Jersey! That all depends on your family dynamics and wishes and choices.

One caveat here, when you get into community programs your chances of getting fellowships later on diminishes significantly compared to the case where you finish a university program.
