Certificate Asking my attending to write the LoR!

I asked one of my IM attending for a letter back in November of last year. She agreed, gave me her email address, and all the info she needs to write the letter. But she told me to give it to her in July when I get my eras.
She is known to write really good letters but a lot of time has passed since then. Should I email her asking if she would still be willing to write one? And how exactly should I ask this?
