Your CS Failure definitely makes your chances less than those who passed the CS from the first attempt.

Is your chances became zero?
Answer: No.

I know many IMGs who failed the CS then got matched.

If you are serious about the US future, then you can do it.

How can I do it?
Super high scores in Step 1 and Step 2 CK
Pass the CS from the second time, don't go for third
Do USCE, externships or observerships
Do research, publication articles
Make connections with residency programs in US

So if you are able to do the above and really determined to spend lots of efforts and time and money, then yes go for it, otherwise cancel the idea if you cannot be that determined.

Yes. Above 240 and if above 250 you will really mitigate the effect of the CS attempt I would say by 80%
