Default Letter Request Form or DSF Signed?

One of my letter writers requested me a Cover Sheet= i assume he means the new Letter Request Form,

So i should send him 2 things (correct me if I'm wrong)

1. Letter Request form (i got it from myEras after finalizing the letter writer)

2. DSF form

Can i send this documents via PDF both of them, and he can print them
and send the 1. LOR 2. DSF 3. Letter Request Form -- all in one package.

My question is:
Can i send the Request Form and DSF forms in a PDF file to the doctor, or do i have to sign any of them and send them printed versions?? please help.

Well after waiting for a reply for a while i decided to call ECFMG. They told me that non of the forms need signature (DSF and LRF) so i emailed them to the Authors.
