Certificate Which LoR to choose?

the one from a US-private practicioner I worked with for 3 months or the one from my boss at my PhD fellow job in my home country (who is also a vice-rector of my medical school)? I have other LORs as well so I have to choose.
Sabio, Steptaker or someone with personal experience please help me!

What are the other letters that you have?

If it has been less than 18 months since you left your home country, then it might be a good idea to put up a letter from there (provided you have 3 other US LoRs)

thank you guys for replying!

my LORs (all waived & all for psych):
1. chief of service at a university-affiliated department where I did observership
2. attending physician from the same place that I closely worked with (I am sure this is very positive so I want to include)
3. associate director of a research center at another US university I did research with.

and the 4th is the one in question. I am still enrolled in the PhD program. In the private practice I had a very active internship.
Is that a good idea to use the US private practicioner's letter from community-based and the one from my boss to university-based programs?

Yes is very good, dont stress your good to go

well as a 'good-old' european tradition, I am supposed to write the LOR for myself cause my boss doesn't want to spend time on it, just wants to sign it at the end. so i am dropping this one.
