Help Observership LoR; Does it Work?

I am an IMG done with Step 1, CK n CS and preparing for my step 3.
I need to add on some good USCE to my profile.
1. People around me are advising that its pretty hard and almost zero chance to find externships. Is this true??
2. I know the importance of US lor's in the interview selection but have a doubt regarding this...... How strong can an observership lor be..... the person giving the lor just mentions we "observed"..... and he may mention few points like punctual, etc. Can this be a strong one for IMG's ??
3. Under the above circumstance is the letter head of the lor that matters most....??
4. How important is the LETTER HEAD of the institution regardless of the type of an lor?? like a just "OKAY" lor to GOOD lor to a STRONG lor.

I appreciate your suggestions...... They wil be of great help.....

Yes. The origin of the letter (letter head) is more important than the content of the letter.

Therefore an observership LoR at Harvard (or any great school for that matter) will have higher impact than an externship LoR at a small non-famous institute.
