what do you mean? IMG going into cardio fellowship directly or IMG doing cardio after US residency?

Former has bigger chance (caveat, you don't get to practice in US, heard they give few yrs for visa to train & after, go back to one's country to practice). The latter, it is quite the norm that cardio is one of the most difficult/competitive fellowship to get into (along with GI & nephro, the most lucrative subspec w/c happens to be among the most difficult to get into)

i must tell you that score isnt enough. for score, it is competitive to have a 240+ score. but you also must have a pro-active medical training background, meaning tonnes of research in cardio, chief resident, excellent feedback on residency & passion for cardio that says it all on your application.

it's not impossible, one just have to be a "superman".. good luck bro..
