Help An IMG requesting your advice

I finished my MBBS in 2007, and worked as a medical writer for 1.5 years in a CRO. I quit my job to come to the US with my husband.

I lived in a town without many opportunities for someone on a dependent visa, but I tried to get a volunteer position, without success. I wrote my GRE and TOEFL in the hope of pursuing a PhD. But I was rejected since I did not have any academic publications/ research experience.

I am planning to write my USMLE by next year and apply for a match.

Realistically, what in your opinion, are my chances at success?
I request you to please be honest and not pessimistic or overly encouraging.
Thank you.
