The USMLE recommends to medical licensing authorities that they should not accept doctors for licensing unless they have passed all the steps (Step 1, Step 2 CK, Step 2 CS, and Step 3) within 7 years time limit.

The ECFMG also implies the same restriction. You won't be ECFMG certified or your ECFMG certificate will be invalidated if you don't pass all 3 steps in a seven year time limit.

For this reason, you might be asked to retake a USMLE Step even if you have passed it before. For example, you passed step 1 exam in 2001, then you passed CK, CS, and Step 3 in 2009. Then you apply for medical license in one of the States, the State medical board will ask you to go and retake Step 1 exam and they'll contact the NBME/ECFMG to allow you to retake that exam even if you have passed before.
