Question What name to use on myERAS?

Posting this for a friend who is applying for next year's match.

His name is Rajesh (first name) Samuel (last name), and the name on his degree certificate is Rajesh Samuel D.S. The name on his Indian passport is Rajesh Samuel (first name) Durai Samuel (last name) since we are instructed in our passports to enter father's name in the last name. Consequently, his ECFMG USMLE score reports also show my name as Rajesh Samuel (first name) Durai Samuel (last name).

Now, when he applies for his residency, he wants to get his name printed correctly which is Rajesh (first name) Samuel (last name). So, is it okay if he enters his name this way in ERAS or does it have to be exactly like it is in the score reports? He does have a letter from the Dean of our medical school saying "Rajesh (first name) Samuel (last name), and Rajesh Samuel D.S are one and the same person".

Yeah. He should use the name that he used with ECFMG and that appears in his score reports.

I don't think that is exactly right. He called up ERAS and they told him that it is okay to use a slightly different name for myERAS.

Even if ERAS is OK with that, he will later have problems with the residency program to where he matches to.

ECFMG records and your USA career record should match exactly.

Apparently, they told him that NBME would use the USMLE ID to connect the USMLE transcript to the ERAS application so there should be no problems connecting the ECFMG score transcripts to the AAMC ID number. What sort of problems are you talking about?

One example is that when you apply for state license they will ask you for your ECFMG certificate and your passport and if there is mismatch they'll reject your license application.

What's the solution then? Register with name+fathername and change it after matching, the license after step 3 and the H1 visa?

Isn't it easier to change one's name on the passport before getting the H1 visa if only to keep the same passport? Also, is it not possible to change the name on the ECFMG certificate after actually getting it? Wouldn't ECFMG be willing to issue a new certificate with the new name?
