Question LoR from a doctor I did not work with?

i am planning to apply for 2014 match.. my step 1 is 239, step 2 ck is in August CS in July.. i have almost 1 yr of exp from home country.. i am planning to apply mostly in IM field ..i have 1 month of US clinical exp in IM which means i have 1 LOR from US and i have one from my home country.. i was wondering if its a possibility that i know a doctor in US i havent worked with him but he's willing to give me a letter of recommendation..
my qs is how bad do you thing it would look to a program director that i haven't work with a Doctor but got the recommendation letter?

it is not looked favourable. If they find out, you are screwed.

Looks really sketchy.... i wouldnt use the letter. Do you know the content of the letter?
